PHYSICIAN SERVICES Physician services, whether furnished in the office, the Enrollee's home, a hospital, or elsewhere, means services furnished by a physician: - Within the scope of practice of medicine or osteopathy as defined in law by the New York State Education Department; and - By or under the personal supervision of an individual licensed and currently registered by the New York State Education Department to practice medicine or osteopathy. Physician services include the full range of preventive care services, primary care medical services, and physician specialty services that fall within a physician's scope of practice under New York State law. FHPlus Appendix K October 1,2001 K-ll The following are included without limitations: - Pharmaceuticals and medical supplies routinely furnished or administered as part of a clinic or office visit. - Physical examinations, including those which are necessary for employment and school. - Health and mental health assessments for the purpose of making recommendations regarding an enrollee's disability status for Federal SSI applications. - Annual preventive health visits for adolescents. - Health screening, assessment and treatment of refugees, including the completion of SDOH/LDSS required forms. - Child/Teen Health Program or EPSDT services which are comprehensive primary health care services provided to children under twenty-one (21) years of age. NURSE PRACTITIONER SERVICES Nurse practitioner services include preventive services, the diagnosis of illness and physical conditions, and the performance of therapeutic and corrective measures, within the scope of the nurse practitioner's licensure and collaborative practice agreement with a licensed physician in accordance with the requirements of the Department of Education. RENAL DIALYSIS. Treatments—these treatments are eligible for benefits if you receive them in a Hospi- tal, a Dialysis Facility or in your home under the supervision of a Hospital or Dialysis Facility.
RENAL DIALYSIS. Services for renal dialysis include all charges for covered home dialysis equipment and covered disposable supplies pursuant to Medicare requirements for group health plans. Benefits will also be provided for dialysis training or counseling.

  • HEALTH PROGRAM 3701 Health examinations required by the Employer shall be provided by the Employer and shall be at the expense of the Employer. 3702 Time off without loss of regular pay shall be allowed at a time determined by the Employer for such medical examinations and laboratory tests, provided that these are performed on the Employer’s premises, or at a facility designated by the Employer. 3703 With the approval of the Employer, a nurse may choose to be examined by a physician of her/his own choice, at her/his own expense, as long as the Employer receives a statement as to the fitness of the nurse from the physician. 3704 Time off for medical and dental examinations and/or treatments may be granted and such time off, including necessary travel time, shall be chargeable against accumulated income protection benefits.

  • Mental Health Services This agreement covers medically necessary services for the treatment of mental health disorders in a general or specialty hospital or outpatient facilities that are: • reviewed and approved by us; and • licensed under the laws of the State of Rhode Island or by the state in which the facility is located as a general or specialty hospital or outpatient facility. We review network and non-network programs, hospitals and inpatient facilities, and the specific services provided to decide whether a preauthorization, hospital or inpatient facility, or specific services rendered meets our program requirements, content and criteria. If our program content and criteria are not met, the services are not covered under this agreement. Our program content and criteria are defined below.

  • Professional Development Plan Professional Development Plan (PDP) refers to plans developed by faculty members addressing the criteria contained in Article 22 and Appendix G.

  • Extended Health Care Plan ‌ The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable extended health care plan.

  • Professional Development Program (a) The parties agree to continue a Professional Development Program for the maintenance and development of the faculty members' professional competence and effectiveness. It is agreed that maintenance of currency of subject knowledge, the improvement of performance of faculty duties, and the maintenance and improvement of professional competence, including instructional skills, are the primary professional development activities of faculty members. (b) Information collected as part of this program shall be the sole property of the faculty member. This information or any judgments arising from this program shall not be used to determine non-renewal or termination of a faculty member's contract, suspension or dismissal of a faculty member, denial of advancement on the salary scale, nor affect any other administrative decisions pertaining to the promotion or employment status of the faculty member. (c) A joint advisory committee consisting of three regular faculty members who shall be elected by and are P.D. Committee Chairpersons and three administrators shall make recommendations for the operation, financing and management of the Professional Development Program.

  • Health Care Savings Plan As provided in this Agreement, eligible ASF Members will participate in the health care savings plan (HCSP) established under Minnesota Statute 352.98, and as administered by the Plan Administrator. The Employer is responsible only for transferring funds, as specified in this agreement, to the Plan Administrator. Subd. 1. All ASF Members who receive severance pay as defined in Section A of this article must participate in the health care savings plan. Subd. 2. All severance pay as defined in Section B of this article shall be transferred to the severed employee's health care savings plan account. At the time of separation, if an ASF Member has an approved exception to participation in the health care savings plan account from the plan administrator, then the ASF Member shall receive this payment in one lump sum payment of cash.

  • Speech Therapy This plan covers speech therapy services when provided by a qualified licensed • loss of speech or communication function; or • impairment as a result of an acute illness or injury, or an acute exacerbation of a chronic disease. Speech therapy services must relate to: • performing basic functional communication; or • assessing or treating swallowing dysfunction. See Autism Services when speech therapy services are rendered as part of the treatment of autism spectrum disorder. The amount you pay and any benefit limit will be the same whether the services are provided for habilitative or rehabilitative purposes.
