Rules Governing Sick Leave. Sick leave will be used only for sickness or injury as described in these paragraphs. A. When the need to use sick leave occurs, a member shall call the DPW Supervisor and report his sickness with as much advance notice as possible. B. Any misuse or attempted misuse of sick leave will be cause for immediate suspension without pay to the employee and filing of “charges” for dismissal. C. An employee who is absent on account of illness for a period of three (3) consecutive days or more is required to furnish the Village Board with a doctor’s certificate for proof of illness. After any five (5) non-consecutive days in a calendar month, an employee who is absent because of illness will also be required to furnish the Village Board with a doctor’s certificate. The Village reserves the right to have the employee examined, at its expense, by a doctor of its choice at any time after illness is reported. D. The Village reserves the right to visit the employee's home on the initial day of sick leave or any day thereafter. The person or persons visiting an employee reporting sick will be the Department Head, his designee, or Village Designee.

A department head shall require written proof of illness from an authorized medical authority at the employee's expense for sick leave use in excess of five consecutive working days by personnel in his/her department. Such proof may be required for periods of less than five consecutive working days where there exists indication of abuse.

Department of Homeland Security's e- Verify system to determine the eligibility of: 1. all people employed during the Contract term to perform duties within Texas; and 2. all people (including subcontractors). E. That representatives of HHSC, AG-MFCU and HHS may conduct interviews of Contractor personnel, subcontractors and their personnel, witnesses, and clients without a Contractor's representative present unless the person interviewed voluntarily requests that the representative be present. The Contractor must not coerce its personnel, subcontractors and their personnel, witnesses, or clients to accept representation by the Contractor, and the Contractor agrees that no retaliation will occur to a person who denies the Contractor's offer of representation. Nothing in the Contract limits a person's right to counsel of his or her choice. Requests for interviews are to be complied with in the form and the manner requested. The Contractor must ensure by contract or other means that its personnel and subcontractors cooperate fully in any investigation conducted by representatives of HHSC, AG-MFCU and HHS. F. That if it is a Home and Community Support Services agency ("HCSSA"), the Contractor will hire Personal Assistance Services and Community Support Services providers chosen by the client or the client's legally authorized representative, if requested, and provided the individual who will provide the services: 1. meets minimum qualifications for the service; 2. is willing to be employed as an attendant by the Contractor; and 3. is willing, and determined competent by the Contractor, to deliver the service(s) according to the client's individual service plan.

  • Use of Sick Leave Sick leave is available the first day of the pay period following the pay period in which it was earned, and is taken in units of one-tenth (1/10) of one hour. Use of sick leave is subject to the approval of the appointing authority. Upon request of an employee, the appointing authority may allow the substitution of three (3) or more days of sick leave for paid vacation, provided the employee furnishes a doctor's statement or other satisfactory evidence that the employee was ill or injured for three (3) or more consecutive days.
