Social Services. For Applicants residing in a state NB is licensed, NB will conduct the home study and post-placement services. If the Applicant resides outside of NB's service area, NB, in cooperation with the Applicant, will select a Local Home Study Agency to conduct direct social services. The Applicant understands and consents to the professional exchange of their information between NB and the local home study agency.
Social Services. Social services" means any facilities, functions, programs or services administered or supported financially or otherwise, by State Government including, but not limited to, the following social services, including services to older people funded by Title IV or Title VI, or their successors or amendments or additions thereto of the United States Social Security Act, as amended, and excepting all other service to older people:
Social Services. Social services shall be provided by an individual who possesses at least a baccalaureate degree in social work from a college or university or a baccalaureate degree in a human services field such as sociology, special education, rehabi- litation, counseling or psychology and shall be available to the extent necessary to meet the needs of patients and their families for care that is reasonable and necessary for the palliation and management of terminal illness and related conditions. An indi- vidual providing social services who does not possess the appro- priate degree as of June 1, 1992 shall have 5 years after June 1, 1992 to obtain the appropriate degree. The duties of the individual providing social services shall include the following:
1. Regularly assessing the patient’s social service needs, implementing the plan of care provisions to meet those needs and reevaluating the patient’s needs and providing ongoing psychoso- cial assessment of the family’s coping capacity relative to the patient’s terminal condition; and
2. Linking patient and family with needed community resources to meet ongoing social, emotional and economic needs;
Social Services. Social services, as provided under Title XX of the Social Security Act shall be available in accordance with the procedures of the state of residence. Illinois residents may apply at the local Department of Human Services office.
Social Services. 32.01 All employees, regardless of age, who have completed sixty (60) days worked, will be registered with the appropriate insurance carriers for the following benefits:

Social Services. The FSCH owners provide the necessary resources and referrals, typically provided by a social worker. Should you need assistance; applying for Healthcare, need PT/OT/ST, need Home Health or Hospice services, transportation or any other services not listed, please let Xxxxxxxx or Xxxxxx know and we will be happy to help.
Social Services. Contractor shall provide 0.5 FTE for provision of Social Services. Position descriptions, hiring and orientation of this employee must include input from CMHP. With direction from the IDT, this employee must provide the following services:

Social Services. Each resident has a Social Worker as part of the team who assists in preparing Advance Directives and addressing adjustment and emotional concerns about living at the facility. The Social Worker assists in clarifying financial questions and serves as the family contact for information and referral. The Social Worker assists in obtaining services such as Support groups, Psychological counseling, Hospice care, Discharge Planning and Home Care Services. The Social Worker serves as the contact with the Designated Representative for the team concerning Advance Directives, Care Planning and on-going issues. Part of the team approach is to identify resident’s spiritual needs, whatever they are. Everyone expresses him/herself differently and we try to meet this part of the resident’s wellbeing through discussion with the resident and family. The Therapeutic Recreation staff and Social Worker are available to discuss individual needs, however at any time the Nursing staff can assist. At different times in life, needs change and it is our goal to be of assistance when needed. Staff will assist families in transportation arrangements and can provide information regarding methods, providers and qualified coverage. However, the facility does not provide transportation service for residents or staff to accompany the resident. Emergency ambulance transportation to the hospital for an acute episode is arranged by the Nurse when the MD directs that the resident be sent to the hospital for care. The facility does not assume financial responsibility for ambulance transport and in some circumstances a fee or co-pay is billed to the responsible party.
