Independent Living Services. Except for ESC programs, the Contractor shall comply with the relevant DCS Independent Living service standard current at the time of service delivery for the Contractor’s licensure category and shall address the full array of Independent Living services and skills described therein. These Service Standards are hereby incorporated by reference and are available in their most current form on the DCS website under Older Youth Initiatives at xxxxx://xxx.xx.xxx/dcs/OlderYouthInitiatives.htm (or successor link). The Contractor shall ensure youth/Child are receiving Independent Living skills education in the residential facility. Independent Living education shall start the day the youth/Child is placed, not waiting until the Child reaches age sixteen (16). The Contractor shall document Independent Living services for youth aged sixteen (16) and over in the monthly report submitted via the National Youth in Transition Database (“NYTD”) portal.

Independent Living Services. Services specifically designed to help adolescents make the transition to living independently as an adult. Services include training in daily living skills as well as vocational and job training. Independent Living Stipend2 Payments made to youth previously in xxxxxx care who are now in independent living arrangements for the purposes of payments for housing, food, etc. Support and other structured services provided to strengthen individual skills and/or provide environmental supports for individuals with behavioral/mental health problems. Services are based on the needs of the individual and include training and assistance. These services normally do not involve overnight care by the provider; however, due to the flexible nature of these services, overnight care many be provided on an occasional basis. Service includes “Supportive In-home Services” licensed by the Department of Behavioral Health and Developmental Services. Services, as defined by State Executive Council policy, conducted by an Intensive Care Coordinator for children who are at risk of entering or who are placed in residential care. ICC providers must be trained in the High Fidelity Wraparound model of care coordination and receive weekly clinical supervision. The purpose of the service is to safely and effectively maintain the child in, or transition/return the child home, to a relative’s home, family-like setting, or community at the earliest appropriate time that addresses the child’s needs. Services must be distinguished as above and extend beyond the regular case management services provided within the normal scope of responsibilities for the public child serving agencies. Services and activities include: identifying the strengths and needs of the child and his family through conducting comprehensive family-centered assessments; developing plans in the event of crisis situations, identifying specific formal services and informal supports necessary to meet the identified needs of the child and his family, building upon the identified strengths; implementing, regular monitoring of and making adjustments to the plan to determine whether the services and placement continue to provide the most appropriate and effective services for the child and his family. A family support partner is part of the High Fidelity Wraparound (HFW) team that offers various levels of support for families based on the family’s needs and HFW plan. Independent Living Services. OKDHS agrees to provide independent living services to tribal custody youth in the same manner as OKDHS youth in accordance with the Xxxxxxx Act of 1999.
Independent Living Services. Each tenant of a Housing Unit that is a recipient of Independent Living Services provided by Developer and other RCOC-monitored agencies pursuant to this Agreement will:
(a) Make their own everyday choices, such as selecting the clothes they

Per DCS’ policy, the current approved assessment tool for children age sixteen (16) and older is the Xxxxxx-Xxxxx Life Skills Assessment, which can be located and completed online at xxx.xxxxxxxxxxxxxxx.xxx. The obligations described herein regarding assessments assume the use of DCS’ currently approved tool. DCS will notify Contractor if the approved independent living assessment tool changes. The assessment is to be completed by the Child as well as his/her caregiver. The Contractor is responsible for administering each assessment, analyzing the results, and carrying out all the recommended services including prioritizing, with the Child's input, the applicable independent living (“IL”) skills that the Child needs to acquire and teaching the skills as prioritized. Contractor shall comply with the relevant DCS IL service standard current at the time of service delivery (posted on DCS’ website, currently at xxxx://xxx.xx.xxx/dcs/2333.htm) and shall address the full array of IL services and skills described therein. These Service Standards are hereby incorporated by reference and are available in their most current form on the DCS website. The Contractor should ensure that the assessment is completed and returned to the referral agent within 30 days of placement or the youth’s sixteenth (16th) birthday. The assessment should be updated annually. Contractor should work with xxxxxx parents to ensure youth are receiving Independent Living skills education in the xxxxxx home. Contractor should ensure xxxxxx parents are able to identify teachable moments and to ensure that these skills are taught in natural settings (i.e., as part of incorporating the Child/youth into the xxxxxx home) and provide experiential learning opportunities accordingly. IL topics of education should include those above. Independent Living Services. Effective July 1, 2010:
a. The Agency social service worker shall conduct a visual and exploratory review of a youth’s living unit at least monthly, to include a review for safety, use of alcohol, and illegal contraband. The Agency shall have a policy regarding actions that will be taken, including notifying the Cabinet social service worker, if there are any safety issues, use of alcohol, or illegal contraband.
b. The Agency social service worker shall document observations of the youth’s living arrangement. These observations should include details surrounding the youth’s progress in living independently.
c. The Agency shall ensure that the youth receives health care as necessary. The Agency shall educate the youth about the importance of good health and assist with scheduling of or transportation to medical appointments if the youth is in need of health care services.
d. In addition to the independent living classes offered by the Cabinet, the Agency shall teach independent living skills to the youth. The Agency shall document efforts to teach these skills as well as document the youth’s progress in mastering these skills.
e. The Agency shall prepare and submit to the Cabinet social service worker a written discharge summary within fourteen (14) days following the date of discharge.
