STATEMENT OF WORK AND SERVICES. By affixing a signature to this contract, the Subgrantee acknowledges receipt of the items listed in this section. The Subgrantee also acknowledges responsibility to perform in a satisfactory manner, as determined by the DCAA, the activities and services authorized by this agreement in accordance with the most current version of these documents: • Contract, • Iowa Weatherization Program’s Policies and Procedures Manual, • Iowa Weatherization Work Standards Manual (which reflects SWS requirements), • Iowa Weatherization General Appendix, • Weatherization Program Notices, • DCAA directives, and • Federal, state, and local laws and regulations Allowable measures under this contract include: Measure1 Minimum Efficiency Level and Performance Criteria (if Applicable) Reimbursement Limit Low-Flow Showerheads 2.0 GPM - max 2 per house $10 each Faucet Aerators 1.5 GPM Brass with Chrome Finish - max 3 per house $5 each Pipe Insulation Rigid 0.5” foam with 0.75” diameter – max 2 6-foot sections per house – 6’ on each side (cold & hot) $10 each Energy Efficient Light Bulbs 5 to 30 Xxxxx ENERGY STAR®-labeled $7 each Programmable Thermostat Must be a 7-day, 5 + 2 day or 5-1-1 day program $100 High-Efficiency Furnace 92+% AFUE or 90%+ for mobile homes– max 1 per house $3,500 Venting for Furnaces $250 each Refrigerator Replacement is indicated based on Baseload Appliance Rating Tool (BART) test – Replacement must be Energy Star Rated - max 1 per house $850 Freezer Replacement is indicated based on (BART) test – Replacement must be Energy Star Rated - max 1 per house $650 Clean and Tune Natural Gas Furnace May be performed on 1 existing natural gas furnace per house, regardless of efficiency level. $125 Infiltration Measures Caulking and sealing of whole house as indicated by initial audit including attic bypass sealing. $500 Duct Sealing Only applicable to ducts in unconditioned space. $350 Insulation Measures Includes wall, attic, floor or foundation, duct, underbelly and bandjoist insulation or a combination thereof2 $4,000 General Repairs In support of successful application of efficiency measures $400 1Company must directly provide the energy (either natural gas or electricity) used by the weatherization measure being installed and the customer must be on a residential rate.

STATEMENT OF WORK AND SERVICES. The Borrower shall perform in a satisfactory and proper manner, as determined by the City, the activities described in the approved application, Attachment "B" to this Agreement. This Attachment is hereby made a part of this Agreement by reference. In addition, Borrower's activities shall conform to the approved Project Budget form in Attachment "B" to this Agreement, and the approved Project Schedule in Attachment "B" to this Agreement.

  • Scope of Works (a) Users with an appropriate licence type may be able to create and access Scope of Works. (b) The parties acknowledge and agree that: (i) any wording contained in a Scope of Works is established by the Customer, is customisable and within the Customer's absolute control; (ii) Users make decisions within ProcurePro on how to draft Scope of Works and ProcurePro is not responsible for those decisions; (iii) the Supplier is not liable for the Customer's use or reliance upon any Scope of Works; and (iv) the Supplier is not responsible for controlling the use, copying, modification or export of a Scope of Works by any User to which the Customer allows access to that Scope of Works.

  • Design Services The Engineer shall perform services during the schematic design phase, the design development phase, the contract documents phase, and the bidding period as hereinafter specified.

  • Hosting Services 13.1 If Supplier or its subcontractor, affiliate or any other person or entity providing products or services under the Contract Hosts Customer Data in connection with an Acquisition, the provisions of Appendix 1, attached hereto and incorporated herein, apply to such Acquisition. 13.2 If the Hosting of Customer Data by Supplier or its subcontractor, affiliate or any other person or entity providing products or services under the Contract contributes to or directly causes a Data Breach, Supplier shall be responsible for the obligations set forth in Appendix 1 related to breach reporting requirements and associated costs. Likewise if such Hosting contributes to or directly causes a Security Incident, Supplier shall be responsible for the obligations set forth in Appendix 1, as applicable. 14 Change Management

  • New Services If, within ninety (90) days after the Distribution Date, a Party desires the other Party to provide additional or different services which such other Party is not expressly obligated to provide under this Agreement (excluding, for the avoidance of doubt, any Additional Services or Service Adjustments, the “New Services”), then such Party will provide a written change request (in the form agreed by the Parties) to the other Party within ninety (90) days after the Distribution Date. The Party receiving such request shall negotiate in good faith to provide such New Service; provided, however, that no Party shall be obligated to provide any New Services, including because the Parties are unable to reach agreement on the terms thereof (including with respect to Service Charges therefor). If the Parties agree to any such New Service, then the Parties shall document such terms in a Service Schedule to be incorporated in Schedule A or Schedule B, as applicable. The Service Schedule shall describe in reasonable detail the nature, scope, service period(s), termination provisions and other terms applicable to such New Services. Each supplement to the applicable Service Schedule, as agreed to in writing by the Parties, shall be deemed part of this Agreement as of the date of such agreement and the New Services set forth therein shall be deemed “Services” provided under this Agreement, in each case subject to the terms and conditions of this Agreement. The Parties shall in good faith determine any costs and expenses, including any start-up costs and expenses, which would be incurred by the Provider in connection with the provision of such New Service, which costs and expenses shall be borne solely by the Recipient.

  • Support and Services ISD and HC agree to the following conditions: A. HC agrees to the following for both the mathematics and English language arts courses: i. To share data and provide feedback regarding student success on entry‐level college mathematics and English language arts courses; ii. To train advisors to recognize and honor course(s) on school district transcripts; iii. To ensure that eligible students are counseled directly into college level mathematics, English language arts, and all other courses that require mathematics and English language arts college readiness; B. HC agrees to the following for the college preparatory mathematics courses: i. To provide the Student Learning Outcomes; ii. To provide the syllabi for the courses being offered. iii. To provide regular meetings between the HC faculty and ISD faculty teaching the course. C. HC agrees to the following for the college preparatory English language arts course: i. To provide the Student Learning Outcomes for Integrated Reading/Writing (INRW 0303) course; ii. To provide the syllabi, including types of essays required (i.e., expository, persuasive, and critical analysis). iii. To provide regular meetings between the HC faculty and ISD faculty teaching the course. D. ISD agrees to the following for both the mathematics and English language arts courses: i. To provide highly qualified instructors for the courses being taught; ii. To identify students who are not college ready as stated in HB 5; iii. To provide professional development and resources required to teach the mathematics and English language arts courses; iv. To identify successful completion of the course(s) on the student transcripts as determined by the State of Texas PEIMS number; v. To provide curriculum for the course that is consistent with HC Student Learning Outcomes; vi. To provide assistance with admission, enrollment, and financial aid applications; E. ISD agrees to the following for the college preparatory mathematics course: i. To teach a math course designed to focus on college mathematics (algebraic or non‐algebraic) concepts; ii. Require students to meet college readiness scores on the TSI Assessment; iii. To meet regularly with HC faculty. F. ISD agrees to the following for the college preparatory English language arts course: i. To teach an integrated Reading and Writing course that focuses on critical reading and college‐level writing; ii. Require students to meet college readiness scores on the TSI Assessment; iii. To meet regularly with HC faculty.

  • Technical Support Services 2.1 The technical support services (the "Services"): Party A agrees to provide to Party B the relevant services requested by Party B, which are specified in Exhibit 1 attached hereto ("Exhibit 1").

  • Work Order (s) means a detailed scope of work for a Service required by Transnet, including timeframes, Deliverable, Fees and costs for the supply of the Service to Transnet, which may be appended to this Agreement from time to time.
