Submission of Absence from Duty Forms. 31 The Employee must submit an Absence from Duty form to the principal or 32 supervisor within forty-eight (48) hours after returning to work after 33 an absence. 35 Unless on an approved leave for childbirth, pregnancy, miscarriage, or 36 abortion, a grant for illness or injury in excess of five (5) 37 consecutive days must be verified by a written statement from a 38 physician including the dates of absence and the release of the 39 Employee to return to work. 41 Illness or disability caused by or contributed to by pregnancy, 42 miscarriage, abortion, childbirth, and recovery there from are 1 considered temporary disabilities and will be treated as any other 2 personal illness or disability to the extent provided by law.

  • Notification of Absence To be entitled to payment in accordance with this clause the employee shall meet the following criteria;

  • ABSENCE FROM DUTY Unless a provision of this agreement states otherwise (e.g. sick leave), an employee not attending for duty will lose their pay for the actual time of such non- attendance.

  • Return from Leave of Absence (a) Before a Nurse may return to work from a leave granted under Article 9.00, she or he must provide a minimum of four (4) weeks written notice of the specific date of his or her return to work, or such shorter time as mutually agreed.

Notification of the approval or denial of their requested leave shall be given to the employee in writing within five (5) working days after the submission of the request.   • ABSENCE FROM WORK AND REPORTING 17.01 If an employee is unable to report for work, he/she shall give the Employer a minimum of four (4) hours notice. In case of day shift work, this time element shall be one (1) hour. If notice is not given within the required time, the employee shall not be entitled to his/her sick pay on the first day of illness.

  • Leave of Absence for College Committees An employee whose assigned work schedule would prevent her/him from attending meetings of a college committee to which s/he has been elected or appointed, will be granted a leave of absence from her/his regular duties without loss of pay or other entitlements to attend such meeting(s). Where such leave is granted, the employer will replace the employee as necessary. Costs arising from this provision will not be charged against the program area of the participating employee.

  • Leave of Absence With Pay 1. A leave with pay will be granted when an employee serves on a jury or is subpoenaed as a witness in court. The employee shall claim any jury, witness or other fee to which he/she may be entitled by reason of such appearance and forthwith pay the same over to the county clerk.

  • Leave of Absence for Union Functions a) Upon request to the Employer, an Employee elected or appointed to represent the Union at conventions shall be allowed leave of absence with pay and benefits.
