Bidder’s Background and Experience. (Section B.1) A. Provide all required documents that demonstrate that the Bidder meets all the minimum requirements stated in Paragraph 3.0 (Minimum Mandatory Requirements) of this WOS. B. The Bidder must complete, sign and date the Bidder’s Exhibit 1 (Organization Questionnaire/Affidavit) as set forth in Appendix B (Required Forms). The person signing the form must be authorized to sign on behalf of the Bidder and to bind the applicant in a WO. C. Taking into account the structure of the Bidder’s organization, Bidder must determine which of the below referenced supporting documents the County requires. If the Bidder’s organization does not fit into one of these categories, upon receipt of the Bid or at some later time, the County may, in its discretion, request additional documentation regarding the Bidder’s business organization and authority of individuals to sign WOs. If the below referenced documents are not available at the time of bid submission, Bidders must request the appropriate documents from the California Secretary of State and provide a statement on the status of the request. The Bidder must submit the following documentation with the Bid: 1) A copy of a “Certificate of Good Standing” with the state of incorporation/organization. 2) A conformed copy of the most recentStatement of Information” as filed with the California Secretary of State listing corporate officers or members and managers.

Co-Occurring Disorders (COD) Related Services; 3. Stigma and Discrimination Prevention; 4. Peer and Family Support Groups; and 5. Outreach and Engagement Bidder must provide narrative describing the services listed here and the major projects/services bidder has provided to the target population. The narrative should include dates and locations where services have been provided, any community partners involved, and any success stories that may be relevant.

  • Background and Security Investigations 7.5.1 Each of Contractor’s staff performing services under this Contract, who is in a designated sensitive position, as determined by County in County's sole discretion, shall undergo and pass a background investigation to the satisfaction of County as a condition of beginning and continuing to perform services under this Contract. Such background investigation must be obtained through fingerprints submitted to the California Department of Justice to include State, local, and federal-level review, which may include, but shall not be limited to, criminal conviction information. The fees associated with the background investigation shall be at the expense of the Contractor, regardless of whether the member of Contractor’s staff passes or fails the background investigation. If a member of Contractor’s staff does not pass the background investigation, County may request that the member of Contractor’s staff be removed immediately from performing services under the Contract. Contractor shall comply with County’s request at any time during the term of the Contract. County will not provide to Contractor or to Contractor’s staff any information obtained through the County’s background investigation. 7.5.2 County, in its sole discretion, may immediately deny or terminate facility access to any member of Contractor’s staff that does not pass such investigation to the satisfaction of the County or whose background or conduct is incompatible with County facility access. 7.5.3 Disqualification of any member of Contractor’s staff pursuant to this Paragraph 7.5 shall not relieve Contractor of its obligation to complete all work in accordance with the terms and conditions of this Contract.

  • Experience A minimum of 1 year of IT work experience in computer systems or support with demonstrated working knowledge of basic hardware and software products and problem solving/troubleshooting skills.

  • Work Experience Formula hour equivalents will be given on the basis of the following number of students enrolled at fourth week census: 5-14 students enrolled = 1 (one) formula hour 15-24 students enrolled = 2 (two) formula hours 25-34 students enrolled = 3 (three) formula hours

  • Background Investigation The BOARD is prohibited from knowingly employing a person who has been convicted of committing or attempting to commit certain criminal offenses. If the required criminal background investigation is not completed at the time this Contract is signed, and the subsequent investigation report reveals that there has been a prohibited conviction, this Contract shall immediately become null and void.
