Coaching Agreement. Welcome to Get Your Head In The Game, a professional coaching practice. This document and attachments constitute a contract (the “Agreement”) between you and Get Your Head In The Game. You should read it carefully and raise any questions and concerns that you have before you sign it.
Coaching Agreement. You have purchased an online course or group coaching program, Weekly content will be delivered via email, the group page on XxXxxxXxxxxxxxx.xxx (user name & password required for login), group coaching calls (to be scheduled) and/or one-on-one coaching calls as needed. Your participation in the online course/group program is governed by the terms and conditions of this Coaching Agreement. By purchasing the online course or group program, you agree to these terms and conditions. The preferred place for questions and support is in the private Facebook group; however you can email me with ideas, problems, questions any time Xxxxx@XxXxxxXxxxxxxxx.xxx or via Facebook chat. I strongly encourage you to share how you are doing while working through the course/program in the Facebook group. Utilize the tribe for ideas, support and encouragement! In order to honor and protect the my intellectual property, you agree not to disclose or communicate information about my practice, course/program materials, fees, or methods to any third parties. You also agree NOT to give content from the online course/group program or any other original content to other parties. I acknowledge that I may obtain confidential personal and/or business information from you and agree to keep and maintain such information confidential and not to disclose or use such information without your prior consent. I encourage you to read my complete privacy policy here: XxXxxx Nutrition Privacy Policy You expressly agree to keep the discussions and content of the private FaceBook group for this course/program strictly confidential, including the identity of other members and any confidential personal information. The private (secret) FaceBook group associated with this online course/group program is closed and only available to members of the course/group. However, Xxxxx XxXxxx / XxXxxx Nutrition is not responsible for any breaches of FaceBook security or the actions of any members of the course/group. Please use your own judgment regarding any information that you post in the FaceBook group. Any members violating the confidentiality policy will be removed from the FaceBook group.
Coaching Agreement. My role as a coach is to listen to you at a deep level, help you explore your options, help you navigate change or transition, and encourage you to look within yourself at a holistic level for answers because you are your best expert. Your role as a client is to be open to exploring the topics you decide to focus on, consider possible goals, and take healthy and positive action towards them. Together we will work as partners to put your best self forward. When in my coaching role, I will encourage you to lead an exploration of your issues, perspectives, dreams, hopes, and desires. If you would like specific input from me, I can switch to a consulting role briefly, and offer ideas or additional information for you to consider—with you being the one who sorts through the information and decides what might be useful for you. D H A R M A G R O W T H , L L C S V E T L A N A D I M O V S K I , P H D , P C C Near the end of the session, I will ask you for the specific action you would like to take as personal work or soul work, for the time between sessions. One way to be successful in completing your goals is to do something called “under-promising” or distilling your goal down to a very reasonable, interesting, and achievable task. I will also ask you for a “take-away” from the session—something you gained during the coaching encounter that you can carry with you between sessions, as part of your developing awareness of your growth and sense of accomplishment.
Coaching Agreement. Coach agrees to maintain the ethics and standards of behavior established by the International Coaching Federation “(ICF)”. It is recommended that the Client review the ICF Code of Ethics and the applicable standards of behavior.
Coaching Agreement. If the Client is currently under the care of a mental health professional, it is recommended that the Client promptly inform the mental health care provider of the nature and extent of the coaching relationship agreed upon by the Client and the Coach.
Coaching Agreement. Coaching fees are paid in advance of the coaching session. The program will not exceed the mutually agreed number of coaching hours. Sessions can be reserved up to 90 days in advance. The Coach requires 24 hours notification about appointment change or cancellation. Clients arriving or calling late will receive their remaining scheduled time only. If a client is more than 15 minutes late, that will be considered a no-show, and the whole session will be deducted from the total number of program coaching hours. These guidelines are in place to honor the valuable time of both coach and client. The Coach will be available to the Client by e-mail and voicemail in between scheduled meetings, as defined and described in the Client Welcome Kit by the Coach. The Coach may also be available for additional time, per the Client’s request on a prorated basis rate of $500/hour. CONFIDENTIALITY This coaching relationship, as well as all information (documented or verbal) that the Client shares with the Coach as part of this relationship, is bound by the principles of confidentiality set forth in the ICF Code of Ethics. However, please be aware that the Coach-Client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not subject to the protection of any legally recognized privilege. The Coach agrees not to disclose any information pertaining to the Client without the Client’s written consent. The Coach will not disclose the Client’s name as a reference without the Client’s consent. Confidential Information does not include information that: (a) was in the Coach’s possession prior to its being furnished by the Client;

Coaching Agreement. Coach I agree to serve as your Coach - to partner with you to identify and achieve your personal and/or professional goals. During the time we spend together in our coaching sessions, I will devote my time, thoughts, and energy to you, exclusively. In between our sessions, I may not be instantly available, as I may be attending to others, or myself. I will however, always attempt to be available within 24 hours. As your Coach, I will bring attentive listening, understanding, belief in you and commitment to your success. You can expect me to challenge you, offer fresh perspectives, make requests (including assigning homework), acknowledge your wins, and guarantee utmost confidentiality (to the fullest extent of the law, and so long as I don't fear for your or another's safety) in the powerful, sacred relationship. I am bound to abide by the IAC Ethical Principles and Code of Ethics, please familiarize yourself with them, they are found here: xxxx://xxxxxxxxxxxxxx.xxx. I am a Coach, not a psychotherapist or physician, and I am not trained in diagnosing psychological or medical conditions. If any issues come up for you that should be handled by a licensed therapist or physician, I insist that you must attend to your health by contacting the appropriate professional. As your Coach, I cannot guarantee results. You will create powerful results by having the courage and determination to forward the action in your life. Throughout the working relationship, the coach will engage in direct and personal conversations. The client can count on the coach to be honest and straightforward in asking questions and making requests. The client understands that the power of the coaching relationship can be granted only by the client, and the client agrees to do just that: have the coaching relationship be powerful. If the client believes the coaching is not working as desired, the client will communicate that belief and take action to return the power to the coaching relationship. Our signatures on this agreement indicate full understanding of an agreement with the information outlined above.
Coaching Agreement. As a client, I understand and agree that I am fully responsible for my physical, mental and emotional well-being during my coaching calls, including my choices and decisions. I am aware that I can choose to discontinue coaching with my current coach at any time.
