Complaint Log. The Contractor shall maintain records and a log of all complaints. The log shall include: the date and time when the Contractor was notified by the City or Customer; the Customer's street address; a description of the complaint; whether the complaint was a Legitimate Complaint; the date and time when the complaint was resolved; and a description of how the complaint was resolved.
Complaint Log. The Establishment’s website will prominently feature the name and contact information for an individual to which any comments about the operation of the establishment may be addressed. The Applicant shall maintain a log for every complaint lodged with the establishment for any issue relating to this Agreement, including, but not limited to, complaints of noise, parking congestion, traffic congestion, security, trash, rodents, incidents, violence, crime, and/or operating hours. The log shall include, for each separate complaint, to the extent provided by any such complainant, the name of the complainant, the date of the complaint, any contact information provided by the complainant, the nature of the complaint, and the response taken by the Applicant or the Establishment. Every complaint shall be kept on file in the complaint log for a minimum of three years from the date on which the complaint was made. Applicant shall make the complaint log available to the Board for inspection and copying upon reasonable advance request.
Complaint Log. There was no complaint received in relation to the environmental impact during the reporting period.
Complaint Log. During December 2017, there was a complaint received by Highway Department on 29 December 2017 and referred to the ENPO on 08 January 2018. Then the ENPO forwarded the complaint by email to the R.E. (AECOM), the Contractor (China Harbour) and the ET (ETS-Testconsult Ltd.) of Contract No. HY/2013/02 at 12:20 on 08 January 2018. The complaint detail was "投訴人投訴於大嶼山東岸路,因港珠澳大橋工程的沙塵問題,部門安排了有關洗街車及吸塵車處理有關沙塵問題,但有關車輛就上述問題的處理成效未如理想。投訴人表示洗街車在清洗有關路面時,只是向路面灑水,令原本的沙塵變成泥漿,但卻沒有清理有關泥漿,道路問題根本沒有根治。另外,有關吸塵車的隔濾亦未如理想,吸塵車吸了地上的沙塵後所噴出來的氣體佈滿沙塵,以致有關沙塵除了未被吸走外,更導致道路沙塵滾滾。要求部門監察有關承辦商,煩請部門跟進及回覆。" The complaint is under investigation and to be reported in the next reporting month.
Complaint Log. The monthly report shall contain a copy of Contractor's complaint log, with initial 2227 date of complaint, a description of how each complaint was resolved, and the date of resolution.
Complaint Log. Franchisee shall maintain a record of all complaints made to Franchisee regarding service. This record or complaint log shall include at a minimum the following information: the name, address, and phone number of the complainant, if known; the date of receipt of the complaint; the subject matter of the complaint, the disposition of the complaint; the date of disposition of the complaint, the date and method of notification to the complainant of the disposition. The records of complaints shall be available to the City Administrator or designee for inspection at any time during normal business hours. A copy of the complaint log for the calendar quarter shall be submitted to the City with the franchise payment for that calendar quarter. Complaint records shall be maintained on the Franchisee’s premises for two years.
Complaint Log. Contractor shall maintain a record of all complaints and correspondence requiring action or response received with regard to City's ambulance service, and Contractor's billing or collection services pursuant to this Agreement, including complaints from patients, their representatives, Medicare/ Medicaid, other insurers, and representatives of City. Complaints regarding ambulance service shall be forwarded immediately to the SAFD Assistant Chief of EMS. A written description of the procedure for handling these complaints and correspondence, which is mutually acceptable to City and Contractor, shall be maintained by the SAFD Assistant Chief of EMS.
Complaint Log. MTA will collect in a centralized and easily searchable electronic format a log of every complaint it, or any of its Contractors, receives (whether formal or informal, and regardless of the format in which the complaint was received) regarding disability-related issues or services provided by MTA to persons with disabilities. The log will include the following information for each complaint: i) name of complainant; ii) name of affected individual (if different from the name of the complainant); iii) physical address, phone number, and email address of affected individual; iv) date(s) of incident(s); v) detailed substance of complaint; vi) name of transportation personnel involved in incident(s); vii) resolution of complaint; and ix) any other information relevant to the complaint. MTA will provide a copy of this log to the Department at the intervals described below for Reporting.
Complaint Log. Contractor shall make and keep records of all complaints regarding missed collections which shall include (a) the date of the complaint, (b) the name and address of the complainant, (c) the time and date the complaint was made, (d) Contractor’s determination, and (e) the date and time when the Complaint was resolved. Collection Equipment All vehicles, facilities, equipment, and property used in the performance of this Contract shall be provided by Contractor. At the start of this Contract, all vehicles used in collection shall be in good operating order and not older than four (4) years (i.e., year model 2017 or newer). At no time shall a vehicle be used for collection that is older than 10 years. All vehicles shall be kept in a clean and sanitary condition with the interior of the cab free of clutter. All collection equipment used under this Contract shall meet all applicable state and federal safety standards. Contractor shall obtain all required operating permits and registrations. Collection vehicles shall be painted in the Contractor’s color schemes. The vehicles shall have the number of the vehicle painted on each side of each vehicle and on the rear of the vehicle in a contrasting color from the body color, the letters to be at least six inches high. No advertising shall be permitted other than the name and address of Contractor. Contractor shall place the appropriate customer service telephone number on all collection trucks. Collection vehicles shall be sufficient to service all Structures at the frequency and level of collection specified in the Contract. Collection vehicles shall be capable of handling, in the safest and efficient method available, the Carts or Containers and material specified for each structure on its route. All such vehicles shall be operated in conformity with the laws of the State of Texas. All vehicles used by management personnel, including route supervisors, shall be equipped with cell phones with voice mail so they can be contacted by the City. Collection vehicles will be equipped with two-way communication devices so that the Contractor’s staff and the driver may communicate during the route collection.
Complaint Log. Manager shall establish a tracking system for complaints received from the Company’s insureds and/or Department of Insurance (referred to as the “Complaint Tracking Log”). The Complaint Tracking Log shall be established and maintained in accordance with applicable laws and applicable rules and orders of any appropriate Department of Insurance or other applicable regulatory body. A copy of a complaint, if received initially by Manager, will be immediately forwarded to the Company. The Company and not Manager, unless instructed to do so by the Company, shall respond to all Department of Insurance or other regulatory body complaints.
