COVID POLICY. By attending the ConnectWise event, you certify that you do not fall into any of the following categories:
(1) Individuals who currently or within the past fourteen (14) days have experienced any symptoms associated with COVID-19, which include fever, cough, and shortness of breath; (2) Individuals who have traveled at any point in the past fourteen (14) days either internationally or to a community in the U.S. that is experiencing sustained community spread of COVID-19; or (3) Individuals who believe that they may have been exposed to a confirmed or suspected case of COVID-19 or have been diagnosed with COVID-19 and are not yet cleared as non-contagious by state or local public health authorities or the health care team responsible for their treatment. DUTY TO SELF-MONITOR. By registering to attend an in-person ConnectWise event, you agree to self-monitor for signs and symptoms of COVID-19 and contact ConnectWise at Xxxxxxxxxxxxx@xxxxxxxxxxx.xxx if you experience symptoms of COVID-19 within fourteen (14) days after attending an in-person ConnectWise event. ConnectWise also encourages you to take action to inform other attendees with whom you were in close contact at the event. Due to the nature of the event, ConnectWise cannot guarantee, and does not represent, that it will or can inform everyone who was in close proximity to a person with a possible COVID case of the potential exposure.
COVID POLICY. If Monroe County closes entry into the Florida Keys within 2 weeks of arrival date due to COVID, Guest may cancel and receive a 100% refund.

b) Décor set by COVID-19 Regulations using gloves & masks
c) Monitor Dancing – Social Distancing and make announcement if necessary – Hand out masks to guests when tables are released
d) Monitor doors and keep closed – Hand out masks at doors if guests are not complying with Mask Mandate.
e) Handout Masks (if mandated) when releasing tables for dinner
f) Handout Masks at Doors while flow is in and outside.
g) Check in with Vendors Hourly-manage DJ for Compliance
h) Confirm that hand sanitizer and masks are available and in place.
COVID POLICY. 4.1 The User has published guidelines for hosting in-person events during COVID-19. The licensee agrees to abide by this policy while on campus.
4.2 In addition, Individuals and groups hosting an event must follow the Center for Disease Control (CDC) and Pennsylvania Department of Health (PADOH) guidance for mitigating the spread of COVID-19. The event host(s) must also agree to monitor public health indicators, adjusted orders and restrictions. Either the University or the event host(s) may cancel if the newest information advises them to do so without penalty of cancellation fees.
COVID POLICY. 5.1. In-person sessions require proof of full vaccination. Student must submit proof of vaccination to Teacher before sessions are confirmed. Additional protocol, such as wearing masks during sessions, may be required at sole discretion of Teacher. Refusal to comply with COVID protocols and policy will result in the forfeiture of series. COVID POLICY. All Occasion Transportation, LLC. vehicles will be cleaned and sanitized before each rental. Our staff will practice governmental recommendations to keep you safe and well. We are not responsible if you contract the COVID-19 virus after a rental with us. For large events, All Occasion Transportation, LLC. may require a 50% deposit in advance to reserve your date and vehicles. Should you cancel within 30 days of your event, this deposit will be non-refundable.
COVID POLICY. Bookings affected by Covid illness or lockdown will be honoured for 1 year from original check in date. T’s & C’s:
a. Credit will be valid for the amount invoiced. Should a different seasonal rate per night apply it is the guest’s responsibility to to pay in the difference.
b. The owner reserves the right to approve the booking.
c. No cancellation will be allowed 14 day or less prior to the check-in date.

CU Ballet acts in accordance with the guidelines of public health agencies at the federal, state, and local levels in addition to the requirements of organizations hosting CU Ballet events, including Champaign Ballet Academy, Krannert Center for the Performing Arts and the University of Illinois, the Virginia Theatre and Champaign Park District. By participating in CU Ballet activities, I (we) agree to adhere to CU Ballet's COVID-19 mitigation measures and understand that those requirements may change during the course of the production. I (we) understand that CU Ballet is taking reasonable steps in an effort to mitigate the risk of transmission of COVID-19, and that CU Ballet is not able to guarantee a COVID-19-free environment or eliminate the chance of infection and associated health risks. By participating in CU Ballet activities, I (we) acknowledge associated risks including exposure to contagious virus such as COVID-19 and I (we) accept and assume responsibility for all such risks.

CU Ballet acts in accordance with the guidelines of public health agencies at the federal, state, and local levels in addition to the requirements of organizations hosting CU Ballet events, such as Champaign Ballet Academy, Krannert Center for the Performing Arts and the University of Illinois, the Virginia Theatre and Champaign Park District. Participants in CU Ballet activities, and in particular those unvaccinated for COVID-19, may be required to wear face coverings over their nose and mouth, including during rehearsals and performances, in addition to practicing social distance and other risk mitigations in an effort to prevent the spread of illness. By participating in CU Ballet activities, I (we) agree to adhere to CU Ballet's COVID-19 mitigation measures and understand that those requirements may change during the course of the production. I (we) understand that CU Ballet is taking reasonable steps in an effort to mitigate the risk of transmission of COVID-19, and that CU Ballet is not able to guarantee a COVID- 19-free environment or eliminate the chance of infection and associated health risks. By participating in CU Ballet activities, I (we) acknowledge associated risks including exposure to contagious virus such as COVID-19 and I (we) accept and assume responsibility for all such risks. I (we) agree to the terms and conditions described herein and will abide by them, with the understanding that failure to do so may result in termination of this agreement and my (my child’s) removal from Champaign Urbana Ballet and/or its productions.
