Proof of Vaccination. In the event that the University of Waterloo brings back or imposes requirements for proof of vaccination, all students living in residence must provide proof of vaccination in advance of their residence move-in date, or upon request of the University of Waterloo, in a form acceptable to the University, as detailed in the University of Waterloo vaccination requirement.
Proof of Vaccination. (a) Proof of vaccination may include but is not limited to a COVID-19 vaccination record card, a copy or digital picture of the vaccination record card, or a print-out from the Oregon Health Authority’s immunization registry.
(b) The Employer shall keep all such records separate from the rest of the employees’ personnel files and shall request that no other medical information be disclosed along with proof of the vaccine. The Employer shall not disclose whether any individual is vaccinated, but may disclose such information if it is de-identified and in the aggregate (i.e. 95% of its employees have been vaccinated).

Acceptable documentation as proof of vaccination required for entry include: ● A physical copy of a Covid-19 Vaccination Record Card issued by the CDC ● A digital copy or image of such card Please make sure the proofs include Front and Back of the card. All details must be legible. You MUST bring a government issued photo ID that matches the details indicated on the proof.
Proof of Vaccination. The District is required to verify the vaccination status of staff as required by DOH and L&I. The District will not maintain copies of actual employee vaccination records and will only release an identifiable verification record where required to do so by court order. The Parties agree that vaccination and face coverings/masks are the most effective tools to prevent transmission of COVID and the parties will immediately bargain implementation of the Governor’s mandate that school staff become vaccinated by October 18, 2021.

The documentation must include the employee’s name, date of birth, vaccination type, date(s) vaccination was received, lot number, and the site or provider the vaccination was received at.

Proof of Vaccination. Proof of vaccination shall include a copy (either photograph, pdf or photocopy) of an individual’s CDC “Covid-19 Vaccination Record Card,” a copy of an employee’s health record showing the vaccine, or a note from an employee’s doctor or other medical professional that an individual has received the vaccine.

Proof of Vaccination. Unit members who require access to District facilities and 25 are fully vaccinated against COVID-19 may provide to the District verification of 26 vaccination status by uploading proof of vaccination to OptimumHQ. 27 28 Proof of vaccination status may be provided by unit members through any one of the 29 following methods:
31 a. COVID-19 Vaccination Record Card (issued by the Department of Health 32 and Human Services Centers for Disease Control & Prevention or WHO 33 Yellow Card) which includes name of person vaccinated, type of vaccine 34 provided and date last dose administered);
35 b. Photo of a Vaccination Record Card as a separate document;
36 c. Photo of the Vaccination Record Card stored on a phone or electronic 37 device;
38 d. Documentation of COVID-19 vaccination from a health care provider; or
39 e. Digital record that includes a QR code that when scanned by a SMART 40 Health Card reader displays to the reader client name, date of birth, vaccine 41 dates and vaccine type. The QR code must also confirm the vaccine record 42 as an official record of the state of California. 43 44 Unit members may retrieve digital vaccination records through CDPH at the 45 following site: xxxxx://xxxxxxxxxxxxxxx.xxxx.xx.xxx/. 46
Proof of Vaccination. Once fully vaccinated with the COVID-19 vaccine, employees must provide the District proof of vaccination and a signed Confidentiality of Medical Information Act release to be physically present on campus in accordance with Section III Proof of Vaccination of attached Vaccination and Immunization Plan.
Proof of Vaccination. In recognition of the diverse personal values (including privacy), the District shall rely on self report of vaccinations. This Agreement will not be implemented in a way that impedes on the District’s ability to provide equal opportunity and access to unit members based on their religious beliefs, personal beliefs, and/or medical exemptions as it relates to vaccinations.
