Proof of Vaccination. In the event that the University of Waterloo brings back or imposes requirements for proof of vaccination, all students living in residence must provide proof of vaccination in advance of their residence move-in date , or upon request of the University of Waterloo, in a form acceptable to the University, as detailed in the University of Waterloo vaccination requirement listed at [link to UW has a Vaccination requirement: xxxxx://xxxxxxxxx.xx/coronavirus/return/vaccination-requirement].

The District will not maintain copies of actual employee vaccination records and will only release an identifiable verification record where required to do so by court order. The Parties agree that vaccination and face coverings/masks are the most effective tools to prevent transmission of COVID and the parties will immediately bargain implementation of the Governor’s mandate that school staff become vaccinated by October 18, 2021.

To ensure employee safety against continuing threats from COVID-19, no later than thirty (30) days after the Ratification Date of this Agreement or within two (2) weeks of the start of employment for a new employee, all employees who have not already done so must provide proof to Lambda Legal that they have been fully vaccinated against COVID-19, including the full series of a single-dose or multi-dose vaccine and any booster shots required or recommended by public health authorities or proof of concrete plans for full vaccination at the earliest opportunity. If further booster shots are required and/or recommended by public health authorities, Lambda Legal will provide employees with no less than four (4) weeks’ notice of any date by which they must receive a booster shot. Proof of vaccination shall include a copy (e.g., photograph, pdf, photocopy, or other record from an online patient portal or account) of an individual’s vaccination record with date(s) of vaccination from the CDC or other health authority or medical professional that Lambda Legal deems capable, including an employee’s doctor. If, and only if, proof of vaccination cannot be established through the above-described copies and records, an employee may offer proof of vaccination through an affidavit attesting to the fact that they have received the required vaccination(s) and/or boosters and are in compliance with Lambda Legal’s vaccination policy.

Once fully vaccinated with the COVID-19 vaccine, employees must provide the District proof of vaccination and a signed Confidentiality of Medical Information Act release to be physically present on campus in accordance with Section III Proof of Vaccination of attached Vaccination and Immunization Plan.

A copy of these records must be provided by the owner prior to the dog being admitted for training. Leashes: All dogs must be leashed and under control when entering and exiting any COLT facility or activity, including training areas and parking lots. A regular leash is required (please, no retractable leashes). Discuss with the trainer the use of appropriate collars, harnesses, etc. Be aware that some dogs may be leash reactive. Use caution in approaching dogs of unknown temperament. General Health: Communicable illness: Dogs that have been ill with any communicable condition (such as kennel cough, Giardia, Canine Papilloma Virus) will not be admitted until pet is symptom-free for a minimum of 10 days or written veterinary clearance is provided. Any health concerns, i.e., coughing, diarrhea, rash, etc., should be brought to the attention of a COLT staff member before the dog attends class. Nails: Dogs participating in agility classes need to have their nails maintained at a reasonable length to ensure that they do not slip when climbing agility obstacles. Fleas: All dogs must be free of fleas. It is highly recommended that dogs be kept on year-round flea prevention.
