Deaths. 1) In the event of a death of a Contract Agency inmate in the County jail, the Contract Agency shall be promptly notified by telephone or fax with a follow-up notification in writing via US mail. Xxxxx County Sheriff’s Office and the Xxxxx County Coroner will investigate the circumstances. The Contract Agency may, if it wishes, join in the investigation and receive copies of records and documents in connection with the investigation.
2) Subject to RCW 68.50.160, any other applicable provisions of law, and to the orders of the Xxxxx County Coroner, the County shall follow the written instructions of the Contract Agency regarding the disposition of the body. Such written instructions shall be provided within three working days of receipt by the Contract Agency of notice of such death. All expenses related to necessary preparation of the body and transport charges shall be the responsibility of the Contract Agency. With written consent from the Contract Agency, the County may arrange burial and all matters related or incidental thereto, and the Contract Agency shall pay all such expenses. This paragraph deals with relations between the parties to this Agreement and is not intended to relieve any relative or other person from responsibility for the disposition of the deceased or any associated expenses.
Deaths. If reasonable notice is given to his/her supervisor, an employee shall suffer no loss of regular pay for a reasonable amount of scheduled time lost on account of death in the immediate family or household of such employee.

In the case of death in the family of an employee, excused time off with pay for scheduled time will be granted as follows: Up to five (5) days of paid leave for a death in the immediate family, defined as: • Spouse (domestic partner) • Parents (including step-parents) • Child (including step-children & children of domestic partner) • Sibling (including stepbrother or stepsister) • Grandchild Up to three (3) days for other covered relatives or household of such employee defined as: • Aunt • Uncle • Niece • Nephew • Grandparent • In-law (including mother, father, son, daughter, brother, sister, grandparents) “Household of employee” includes persons who regularly make their home with the employee as a part of the family. Such time off will begin on the day of the death and may extend through the day following the funeral. The Company shall be given reasonable notice prior to intended absence or funeral leave. In no case will payment be granted in lieu of time off nor will payment be made if death and funeral occur during non-work time. If a death or funeral of a member of the employee’s immediate family occurs during the employee’s vacation, the employee will be allowed to use funeral leave and reschedule the remainder of his/her vacation if time and service requirements permit.
Deaths a. Death in the immediate family which requires the presence of the employee. Five (5) duty days allowed per occurrence. Immediate family is defined as anyone living in the household, and/or father, mother, spouse, son, daughter, father-in-law, mother-in-law, son-in-law, daughter-in-law, brother and sister.
b. Death in the non-immediate family, which requires the presence of the employee, with a limit of three (3) days allowed per occurrence. Non- immediate family is defined as grandparents, grandchildren, aunt, uncle, cousin, nephew, sister-in-law, brother-in-law and niece. If travel time is involved, time may be requested under 5 below.
c. Death of a close personal friend, which requires the presence of the employee, with a limit of one (1) day allowed per occurrence.

All AEs resulting in death either during the study period or within 12 weeks (84 days) after the last dose of IMP, must be reported as au SAE withiu 24 hours of the iuvestigator's kuowledge of the event. The convention for recording death is as follows: • AE term: lead cause of death (e.g. multiple organ failure, pneumonia, myocardial infarction) • Outcome: fatal. The only exception is if the cause of death is unknown (i.e. sudden or unexplained death), in which case the AE term may be “death” or “sudden death”.

Such written instructions shall be provided within three working days of receipt by the Contract Agency of notice of such death. All expenses related to necessary preparation of the body and transport charges shall be the responsibility of the Contract Agency. With written consent from the Contract Agency, the County may arrange burial and all matters related or incidental thereto, and the Contract Agency shall be responsible for all such expenses. This paragraph defines the obligations between the parties to this Agreement and shall not affect the liability of any relative or other person for the disposition of the deceased or any expenses connected therewith. Notwithstanding the above, all expenses related to the death of a Contract Agency Prisoner in the County jail resulting from physical injuries incurred while under the care, custody, and control of the County shall be the responsibility of the County.
