Documentary Proof. (i) Proof of qualifications and all prior professional experience must be submitted to the Board. The Employer will inform the Employees, at the time of hiring, that supportive documents must be submitted to the Board. (ii) Supportive documents applicable to allowances as provided for in Article 9 must be submitted to the Board in order to receive payment of the allowance. However, the payment shall be effective as of the date of the course completion or the date of employment with the Board.

Documentary Proof. The Registration Policies set out the documentary proof that you are required to provide to us to establish that you meet the eligibility requirements for the type of Domain Name for which you are applying. We may require you to submit documentary proof to verify any information contained in your application or which we may otherwise require to be provided to us from time to time, even after the registration of your Domain Name. We will determine if the documentary proof you provide is adequate in relation to each application. You may submit copies of the documents that accompany your application, including but not limited to documentary proofs for eligibility requirements, to us by fax, post or electronic means. If you send documents to us electronically, we will only accept documents in .pdf, .gif or .jpg format.
Documentary Proof. The City reserves the right to require the Applicant to produce documentary proof of any matter relating to this Application and/or the Special Event at any time and in any form, and the Applicant shall submit such requested documentary proof within five (5) calendar days of such request.

For the purposes of confirmation of the country of origin in the State Party in order for the free trade regime to be granted, it is necessary to provide the customs authorities of the importing country with the original certificate. The certificate validity for granting of the free trade regime is 12 months from the date of its verification by the authority, empowered in accordance with the legislation of the State Party to issue and (or) verify certificates of origin of goods. 2. Certificate is executed and issued for one consignment. 3. Certificate is provided to the customs authorities of the importing country in hard copy in english or russian. 4. The actual number of delivered goods shall not exceed the amount specified in the certificate for more than 5 percent. 5. The certificate may be issued also after the exportation of goods on the basis of a written request of the applicant. In this case, the applicant shall additionally submit to the authority a customs declaration with the appropriate xxxx of the customs authority, confirming the actual exportation of the goods. The xxxx of the customs authority must antedate the date of issuance of the certificate. In this case, in section 5 of the certificate states: "Issued retrospectively". 6. In case of loss or damage to the certificate its officially certified duplicate is issued. When issuing a duplicate in section 12 "Certification" is indicated the date of issuance of duplicate, and in section 5 "For official use" ("For official use") the word "Duplicate" ("Duplicate") along with the number and date of the lost or damaged original certificate shall be indicated. A duplicate certificate shall take effect from the date of issuance of the original certificate. The validity of a duplicate certificate for the purposes of granting of the free trade regime can not exceed 12 months from the date of issuance of the original certificate. 7. In return of the canceled for any reason certificate or, if necessary, for the re-execution of a previously issued certificate, a new certificate may be issued on the basis of a written justification of the applicant. In this case, in section 5 a record "Issued instead of certificate form CT-2" is made, indicating the number and date of the canceled (re-executed) certificate. Certificate issued in exchange for another certificate is assigned with a new registration number.

Provided that proof of qualifications and experience is submitted within the same school year or calendar year of the date of commencing employment with the Board (“the commencement date”), whichever is longer, the Teacher shall receive any salary adjustment retroactive to the commencement date; otherwise any salary adjustment shall become effective when proof of qualifications and experience is submitted to the Board. In extenuating circumstances, and at its discretion, the Board may extend the period referred to above.

Documentary Proof. Subject to of the Education Act it is understood that no person shall be employed in a secondary school to teach or to perform any duty for which membership in the College is required under this Act, unless the person is a member of the College of Teachers. Newly-hired Teachers shall join the Dental Plan selected by the Board if they are eligible as defined by the Plan. Notwithstanding Section no Teacher shall be required to join the Dental Plan selected by the Board if is able to obtain dental plan coverage equal to or greater than the Board Dental Plan.
