Effective Dates of Coverage. A. Health insurance coverage is effective the employee’s first day worked. Insurance premiums are not pro-rated, but do fall under the 15/16 rule for as long as the health insurer allows this rule. The rule applies as follows. If the effective start date falls on or before the 15th of the month, the full monthly premium will apply. If the effective start date falls on or after the 16th of the month, no premium will be charged.
B. Health insurance coverage ends on the employee’s effective last day of work if the effective end date falls on or before the 15th of the month, and no premium will be due. If the effective end date falls on or after the 16th of the month, the full monthly premium will apply and coverage will extend to the end of the month. Employees who work less than twelve months and whose resignation/retirement is effective at the end of the school year will receive health insurance through July 31.
Effective Dates of Coverage. 7.5.1 If an employee applies within thirty days of date of employment, coverage will begin on the first day of the month coinciding with, or following, the date of employment.
Effective Dates of Coverage. We will provide coverage to you beginning on the day we receive your signed Enrollment Form and the required annual Premium. Such dependents cannot be re-enrolled until the next open enrollment period. Coverage may be continued while on an approved leave of absence without pay as long as the employee pays the total applicable premium cost at the group rate.

To implement the Insurance Program agreed upon herein, the company shall enter into contracts of insurance with insurance companies or service plans where applicable under the terms of which insurance contracts or agreements the insurance companies or service plans as the case may be, shall contract to provide for participants the benefits described in the insurance program.
