Employee Documentation. If required by the COUNTY for a Project and/or Service, CONTRACTOR shall provide the COUNTY’s Project Manager with all requested documentation for all personnel, subcontractors, and representatives of CONTRACTOR that will be utilized for the Project and/or Service. Documentation shall be provided within five working days of request and shall be submitted electronically in PDF format. This information shall also be provided when new personnel, subcontractors, and representatives of CONTRACTOR are hired at any time during the contract period for the Project and/or Service. The information supplied will be used to run background checks and to provide identification badging, proximity cards, and keys. All documentation required below shall be supplied in (1) one PDF attachment that shall be titled with the Company’s name, the person’s name, and the person’s birthdate. Example: ACME Plumbing - Xxxx X. Xxxxx - 10/10/96. The documentation shall include the following: Full name, Address, Email address, Telephone number, Copy of driver’s license/state of Florida identification card/valid passport/valid work visa, current color photo (head shot) taken with a plain background, Building name(s) and address(s) of the facilities where the individual will be working, and any additional information that may be requested by the Lake County Sheriff’s Office.

Other documents submitted by third parties shall be available to the employee.

Employee Documentation. A. Service Provider guarantees that the individuals employed by the Service working on this project are authorized to work in the United States. The Service Provider will upon demand provide Dublin with appropriate documentation (Form I-9) for any Service Provider employee performing services for Dublin. B. The Service Provider agrees to indemnify Dublin in accordance with Section VI of the Agreement for any issue arising out of the Service Provider’s hiring or retention of any individual who is not authorized to work in the United States.
