Health Certificate. A premium can be collected and insurance can take effect under this Limited Insurance Agreement (the “Agreement”) only if the following statement is true: I certify and affirm that the proposed insured has not:
(1) Within the past 90 days been hospitalized or been advised by a member of the medical profession that he or she needs hospitalization for any reason (other than for normal pregnancy or well-baby care).
(2) Within the past 12 months received treatment or advice from a member of the medical profession for heart disease, chest pain, stroke or cancer (except skin). Person proposed for coverage: Amount of insurance requested: $ Amount of prepayment: $
Health Certificate. 32 All employees in positions covered by this contract who fail to provide such health certificate as may 33 be required by State or local law or regulation, shall not be allowed to work under the terms of this 34 contract until said certificate is provided to the administrator of personnel. It shall be the responsibility 35 of the employee to meet and maintain qualifications under this section. The District, if at all possible, 36 will notify all employees in writing at least forty-five (45) days prior to the termination of any existing 37 health certificate, of the pending termination of said certificate, and the employee's inability to work 38 for the District without such a certificate. 40 Section 7.11. 41 Employees required to attend District meetings and/or training sessions will be compensated at their 42 regular hourly rate.
Health Certificate. Prior to signing on the employment contract, the seafarer shall be interviewed and medically examined for fitness on the Employer's account. During the time of employment, the seafarer shall be liable to medical examination for when requested by the Employer or by his representative, at the Employer's account.

DKK 150 will be paid to cover visits to a doctor after the commencement of the employ- ment in connection with the issue and/or renewal of a health certificate. Under applicable law, food enterprises are under an obligation to ensure that food is not contaminated with disease-producing microorganisms. Out of consideration for the operation of the enterprise, the enterprise may enquire into the health status data of employees who are in contact with food during their work. The purpose of the health status data is to ensure that the new employee does not suffer from sickness, is the carrier of sickness or has had a sickness that can be trans- ferred to food. The obligation to disclose all relevant information is limited to the above matters. If the enterprise is to keep the health status data, the employee must give his or her consent to such storage. The data cannot be stored for longer than what is necessary due to the operation of the enterprise.
Health Certificate. All employees of the Board of Education shall meet requirements of Federal and State Laws. New employee contracts will be effective conditional passing a physical. All employees must have a one time physical including a tuberculin test on file in the board office. (Physical Form KSA 72-5213). The Board will pay the actual cost of the employee’s physical. The district will cover the co-pay cost and any other bills associated with a regular physical. (Updated 2023-24) If at any time there is reasonable cause to believe that any such person is suffering from an illness detrimental to the pupils or other employees, the Board of Education may require a new certification of health.

Health Certificate. The Breeder at their sole expense shall obtain a current official certificate of veterinary inspection prior to shipping. The term "official certificate of veterinary inspection" means a legible certificate of veterinary inspection signed by the examining veterinarian licensed by the state of origin and accredited by the United States Department of Agriculture, that shows the age, sex, breed, color, and health record of the puppy, the printed or typed names and addresses of the person (breeder) from whom the animal was obtained, the seller, the purchaser, and the examining veterinarian, and the veterinarian's license number. The official certificate of veterinary inspection must list all vaccines and deworming medications administered to the puppy, including the manufacturer, vaccine, type, lot number, expiration date, and the dates of administration thereof, and must state that the examining veterinarian warrants that, to the best of his or her knowledge, the animal has no sign of contagious or infectious diseases and has no evidence of internal or external parasites, including coccidiosis, giardia, ear mites, fleas and ticks. The examination must include, but not be limited to, a fecal test to determine if the puppy is free of internal parasites, including coccidia, hookworms, roundworms, tapeworms, and whipworms.
Health Certificate. 10.1. The customer declares to have completed the PAR-Q form truthfully and to have informed BeneFIT Studio of all medical data and history.
10.2. The customer is obliged to report changes regarding medical data to BeneFIT Studio.
Health Certificate. Any teacher returning from an extended leave of 3 or more days due to a medical reason shall, at the Superintendent’s discretion, present a certificate from their personal physician or the school physician stating that they are physically fit to return to normal teaching duties.
