Extended Health Insurance Plan. The Company will provide an Extended Health Insurance Plan and pay 100% of the insurance premiums for the following benefits: • Managed Health Care Drug Plan at 100% of all eligible expenses for the Drug Benefit (does not include any drug that is available on an over-the-counter basis whether or not prescribed), and the Supplementary Health Care Benefit. Maximum dispensing fee payable is $8.00 • Vision Care at $200 every twenty-four months • Hearing Aids at $1000 every ten years • Supplementary Health Care (Excluding Semi-Private hospitalization coverage) Positive enrolment - all employees will be provided with enrolment forms to identify the names of dependants and their birth dates who are to be provided with dependant coverage. Benefit claims for dependants will initially be claimed by the spouse whose birthday occurs first in the year when the spouse has coverage available from another source.

Extended Health Insurance Plan. I. The Company agrees that Extended Health Insurance will be made available to participating employees in the bar- gaining unit, as outlined in this Xxxxx- xxx, during the term of this Agreement. Benefits under this Plan are those in- cluded in the Confederation Life Extended Health Insurance Plan, as described in the literature attached to this Appendix and made part hereof. Participation by active employee in this Plan shall be on a voluntary basis. The Company will pay one hundred per cent ) of the premiums for this plan during the term of this Agreement for each active employee in the unit (including his eligible dependents) enrolled through the Plan group. No person will be insured as an em- ployee and also as a dependent under this Plan. APPENDIX VACATION PLAN HOURLY EMPLOYEES e following Vacation Plan applies to all paid on an hourly basis. VACATION SCHEDULE An employee must complete 1 year of Company Service Credit to attain initial va- cation eligibility. After attainment of such eligibility and during the remainder of that calendar year, he shall receive 2 weeks of vacation. During the calendar year in which an employee completes 2 years of Company Service Credit, he shall receive 2 weeks of vacation. During calendar years in which an employee completes from 3 to 9 years of Company Service Credit, he shall receive 3 weeks of vacation. During calendar years in which an employee completes from to years of Company Service Credit, he shall receive 4 weeks of vacation. During calendar years in which an employee completes from to years of Company Service Credit, he shall receive 5 weeks of vacation. During calendar years in which an employee completes or more years of Company Service Credit, he shall receive 6 weeks of vacation. An employee with or more years of Company Service Credit may carry forward to the succeeding year full week of Cur- rent Year Vacation. An employee with or more years of Company Service Credit may carry forward or 2 full weeks of Cur- rent Year Vacation to build towards a maximum accumulation of 3 weeks carried forward vacation credit. An employee with or more years of Company Service Cred- it may carry forward 2 or 3 full weeks of Current Year Vacation to build towards a maximum accumulation of 6 weeks carried forward vacation credit.

Dental Care The will provide a Dental Care Insurance Plan and pay of the insurance premiums for the following benefits: • Basic restorative preventative at reimbursement • Dentures and denture repairs at reimbursement • Crowns and Bridges at reimbursement • Oral recall nine months Provide for the Life The Dental Fee Guide shall be the Suggested Fee Schedule effective first of the month following the date of ratification.
