Fee-based Services. The State Board will ensure that any fee-based services provided by the State Board are set forth in a services agreement that is separate from the charter agreement and ensure that the purchase of such services is not and will never be a condition of charter approval, continuation, modification or renewal.
Fee-based Services. To establish a relationship between the County and the Agency whereby the Agency provides the County the following “Fee-Based Services”: Training for Parents/Staff; Technical Assistance for Head Start Staff; and Individual Service Plan Meetings.
Fee-based Services. The following services (the “Fee-Based Services”) are offered by the Clinic, but involve additional expenses not included within the Membership Fees and incur additional charges. All charges for Fee-Based Services are due and payable at the time the service is provided.

b. Cryotherapy (freezing) for wart treatment
c. Cryotherapy (freezing) of skin lesions
d. Shave skin biopsies
e. Skin tag removal
f. Simple laceration repairs (i.e. stitching or skin glue for minor cuts)
g. Suture removal
h. IUD removal
i. Incision and drainage of minor abscesses j. Nebulizer treatments
Fee-based Services. The Commission will ensure that any fee-based services provided by the Commission are set forth in a services agreement that is separate from the charter agreement and ensure that the purchase of such services is not and will never be a condition of charter approval, continuation, modification or renewal.
Fee-based Services. Certain portions, Services, components, Content and features of the Web Site are only available to users who purchase a subscription (“Fee-Based Services”). As a subscriber to one of the Fee-Based Services, you agree as follows:
Fee-based Services a) On-Site Nurse Navigator Parents and Parents-in-Law will have access to an Onsite Nurse Navigator to attend physician appointments relating to their cancer diagnosis. This personalized one-to-one service helps to ensure nothing is left to chance and all questions and concerns are addressed. A cancer diagnosis is required for the Onsite Nurse Navigator. Parents and Parents-in-Law will have access to an Onsite Nurse Navigator appointment for an additional per appointment fee.
b) Cancer DNA Testing Parents and Parents - in - Law of Participants and Dependent will have access to comprehensive DNA testing for cancer if they become diagnosed with cancer while enrolled in the Cancer Guardian program. Testing is subject to treating physician approval based on clinical need and necessity. Report results are delivered back to the treating physician via secured physician portal in 10-14 days from the date that the test specimen is received at the lab. Complex cases may require additional time for report delivery.
Section C - Cancer Incidence Procedures
Fee-based Services. As an alternative, and for an additional fee, service may be available Outside of Principal Period of Maintenance subject to the availability of personnel, among other factors. The service alternative you have chosen is recorded on your invoice. This service does not cover any related network problems or any Service which would be unique to the System’s operation on a network. This Agreement will only involve such Services as are required to restore your System’s operational capability. For purposes of this Agreement, all references to time mean the customer's local time. References to the United States include the continental United States, Alaska, Hawaii. PPM means the principal period of maintenance or the principal hours during which Services are On-Site/In-Home Service. Subject to the terms of this Agreement, On-Site Service is available for your System within the United States. The technician will need to have the full address of your System's location and know whether it is a residence or business. A service technician will be dispatched to your location to service your System, according to your service level, as follows: Holidays: Regular holidays shall include New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day, unless you are notified otherwise by QUALXSERV. An Adult Must Be Present At Residences: For On-Site Service provided at a residence, an adult must be present at all times during the service technician’s visit.
Fee-based Services. If you subscribe to any fee-based FieldView Services, your terms of service include these general Terms of Service, plus any Additional Terms relating specifically to such fee-based services disclosed to you at the time of subscription, including but not limited to the prices, charges, and terms and conditions provided to you in marketing and informational material associated with the FieldView Services or on the Climate website, all of which are hereby incorporated by reference. Your order for fee-based FieldView Services will become effective only if we accept your order. You agree to pay when due, all amounts set forth in the applicable order for any fee-based FieldView Services and, at the time of renewal, the applicable subscription fees for any renewed FieldView Service subscription, plus applicable taxes (unless you provide us with a valid tax exemption certificate authorized by the appropriate taxing authority). If your subscription fee payment is overdue, we may suspend your account access until such amounts are paid in full. Unless otherwise specified, overdue amounts will accrue interest at the lesser of 1.5% per month or the highest rate allowed by law.
Fee-based Services. Xxxxxxxx for any fee-based services will be in accordance with separate written agreement or contract(s).
Fee-based Services a) Second Deliveries To the extent Exhibit C attached hereto lists the frequency of deliveries as 1.5 per week, there shall be [***]. Additionally, stores with purchases of [***] or more per week can request a second delivery at no additional charge. Stores with purchases of less than [***] per week can request a second delivery at a cost of [***] per week (or [***] per delivery).

c) Tote & Crate Returns Totes and crates (dairy) are detailed on our Driver’s load list. An inbound and outbound count will be verified and signed for by the store employee and driver at the completion of the delivery. This is a memo billing process at the time of delivery. Reports will be run at month end and any serious out of balance will be brought forward with the attempt to collect the actual totes or crates. Store will be billed [***] per tote or crate if the out of balance remains.
