Individual Service Plan. 8.4.1 Both the NCM and SWCM will jointly develop and implement a patient-centered ISP for each client based on discussion of patients’ medical and non-medical needs. The ISP, in conjunction with the client or client's representative, will be developed within seven days of enrollment and utilize the principles of Specific, Measurable, Attainable, Relevant, Time-Bound (SMART) objectives.
Individual Service Plan. You must meet with the ‘inhimiyu’nm Coordinator to work on your Individual Service Plan specific to your home schedule and other productive time.
Individual Service Plan. A. The parties understand and agree that all service provided to eligible recipients under the terms of this contract must be in accordance with the Coordinated Supports and Services Plan (CSSP) developed by the individual client’s social worker or case manager.
Individual Service Plan. 10.1.1. Individual Service Plans must be developed in compliance with WAC 000-000-0000 (for mental health), 000-000-0000 (for substance use residential treatment services) and 000-000-0000 (for substance use outpatient treatment services). The Contractor must require that Individuals are actively included in the development of their individualized service plans, advance directives for psychiatric care and crisis plans. This must include but not be limited to children and their families (e.g. caregivers and significant others, parents, xxxxxx parents, assigned/appointed guardians, siblings).
Individual Service Plan. The parties understand and agree that all services provided to eligible recipients under the terms of this contract must be in accordance with the Individual Service Plan developed by the County with the individual client. Performance of the Provider will be monitored in accordance with client outcomes, goals, and indicators as specified in the Individual Service Plan (ISP). The County may delegate the development of Individual Service Plans to the Provider. If the responsibility is being delegated, the Agency must ensure the development of the ISPs.
Individual Service Plan a. The parties understand and agree that all services provided to eligible clients under the terms of this contract shall be in accordance with the individual service plan developed with the individual client.
Individual Service Plan. (ISP) is the comprehensive plan developed by the CFT that is person-centered and includes the child’s treatment goals and objectives, methods of measurement, the timetables to achieve those goals, a description of the services to be provided, the frequency and intensity of each service, and which service providers will provide each service. This term is synonymous with the WV Wraparound POC.
Individual Service Plan i. Contractor shall submit all completed individual service plans at the end of each Quarter.
