Form of Proposal. Each Bidder shall submit the Bid in four parts (original Bid Form, marked original, with attachments and three (3) copies of the Bid Form and all attachments) and indicate his base Bid price and any alternative(s) that may be included in the proper space(s). The base Bid is the sum of all pay item totals and the County reserves the right to correct errors in pay item totals arising from incorrect extensions. See "Bid Errors", Item 5.
Form of Proposal. All bids must be made on the attached form of proposal. Bid blanks must be completed and clearly filled in and must be free from alteration either by erasure or interlineations, or otherwise the bid proposal will be voided. Bids must be properly signed in ink by the bidder or by an authorized official or agent when the bidder is a firm or corporation. When the bid is made by a firm, the signature must include the firm name, and the signature of member thereof. When made by a corporation, the signature must contain the name of the corporation followed by the signature of the official or person authorized to bind it in the matter and with proof of his authority. When filed, the bid with the accompanying bid security must be enclosed together in a sealed envelope, clearly marked on the outside with the bid number and project name, addressed to the City Clerk, Sullivan, Missouri. The bidder shall designate on the bid blank his official address to which all communications can be mailed. No facsimiles will be accepted.

Form of Proposal. 3.1 Each Proposal shall be on a Proposal Form forming part of the Proposal Documents with the Blank spaces filled in. The Proposal price must be written in words as well as figures, and must be for a sum in Canadian Dollars including all tariffs, freight, duties and taxes other than the Goods and Services Tax which must be shown as a separate amount unless otherwise specifically stipulated. In the event of a discrepancy between an amount written in words and an amount written in figures, the amount written in words shall be deemed the intended amount. Not withstanding the foregoing, the Moose Jaw Downtown & Soccer/Field-House Facilities Inc. shall be entitled to accept a Proposal in such form as the Moose Jaw Downtown & Soccer/Field-House Facilities Inc. in its sole discretion deems acceptable.
3.2 The Moose Jaw Downtown & Soccer/Field-house Facilities Inc. shall not be obligated to accept Proposals that are unsigned, incomplete, conditional, illegible, unbalanced, obscure, or contain irregularities of any kind.
3.3 Proposals shall not be withdrawn, modified or clarified after being deposited with Moose Jaw Downtown & Soccer/Field-House Facilities Inc. unless such withdrawal, modification or clarification is made in writing by signed facsimile and received by Moose Jaw Downtown & Soccer/Field-House Facilities Inc. prior to the time and date specified for the closing of Proposals. Any withdrawal, modification or clarification of the Proposal must be followed by a letter of confirmation signed and sealed in the same manner as the Proposal and delivered to the address for the Moose Jaw Downtown & Soccer/Field- House Facilities Inc. in the Invitation to Bid within five (5) working days of the notice of the withdrawal, modification or clarification.
3.4 Proponents shall provide the information outlined in the Evaluation & Award Criteria forms relevant to the Proposal.

Form of Proposal.
