General Surgery. Surgery charges are covered when treatment is received at an inpatient, outpatient or ambulatory surgery facility, or doctor's office. Your health plan will not pay separately for pre- and post-operative services. Morbid obesity treatment Your health plan covers treatment of morbid obesity through gastric bypass, or other methods recognized by the National Institutes of Health (NIH). Coverage is restricted to surgical procedures and does not include weight control dietary supplements. According to the NIH guidelines, gastric bypass surgery is effective for the long-term reversal of morbid obesity for a patient who: • weighs at least 100 pounds over or twice the ideal body weight for frame, age, height, and gender as specified in the 1983 Metropolitan Life Insurance tables; • has a body mass index equal to or greater than 35 kilograms per meter squared, with comorbidity or coexisting medical conditions such as hypertension, cardiopulmonary conditions, sleep apnea, or diabetes; or • has a body mass index of 40 kilograms per meter squared, without such comorbidity. As used above, body mass index equals weight in kilograms divided by height in meters squared. Coverage does not include weight control dietary supplements or weight loss medications, unless such supplements are recognized by the National Institutes of Health as effective treatment for the long-term reversal of morbid obesity for covered persons meeting the requirements specified above. Reconstructive breast surgery and mastectomy Mastectomy, or the surgical removal of all or part of the breast, is a covered service. Also covered are: • reconstruction of the breast on which the mastectomy has been performed; • surgery and reconstruction of the unaffected breast to produce a symmetrical appearance; and • prostheses and physical complications of all stages of mastectomy, including lymphedemas, in a manner determined in consultation with the attending physician and the covered person. • Reconstructive breast surgery done at the same time as a mastectomy or following a mastectomy to reestablish symmetry between two breasts. General Surgery. NHS Tayside provides General Surgery and some specialist tertiary surgery across three sites at Ninewells, Perth Royal Infirmary (both elective and emergency) and Stracathro (elective only). As per Orthopaedics, key challenges include impact of unscheduled care leading to unpredictable elective bed availability, resulting in elective cancellations and replication of staff/equipment across three sites. Urology NHS Tayside’s Urology day cases are currently split across Ninewells, Perth Royal Infirmary and Stracathro , with high demand in Ninewells and Stracathro . Patients are distributed between three sites based on clinical status and dependent on anaesthesia required, but major procedures are done at Ninewells only. The proportion of patients who are assessed as “unsuitable for Stracathro” is even higher amongst urology patients as they are generally older and frailer than the orthopaedic or general surgery population. This makes scheduling these patients into Ninewells or PRI lists very challenging.

State Secretary and others of Punjab through the (X. X. Xxxxx, X.) 183 do the Postgraduation in the speciality in which he/she is posted. Annexure X-x has been filed along with the return which lays down the criteria for selection of candidates for admission to M.D./M.S. and various Diploma Courses. In the said criteria, rule 5 provides that all the courses are whole time and no candidate can hold any appointment during the course of his studies. If any candidate joins any service after taking admission to the course, his admission to the course will stand automatically cancelled and his fees forfeited. Then reliance was placed on rule 8 thereof which further provides that a full time Postgraduate student means a student who is not doing any job anywhere except in the depart ment concerned as Demonstrator/Registrar/Lecturer or is a full time stipendary in the department concerned. According to the learned counsel for the respondents, reading the two rules 5 and 8 together, it is evident that since the petitioner was working as Demonstrator he could only be considered for the subject in which he was working as Demonstrator and not for any other subject.
