ID Card. Licensee must present their student University ID card (“PolyCard”) or government issued picture identification when requested for access to the housing facilities and for use of the Dining Plan. Presenting fabricated, falsified, or misrepresentative ID; permitting others to use any licensee’s PolyCard for the purpose of improperly gaining access to Residence Halls, rooms, Apartments, Dining Plans, use of equipment, or any other University service or facility is prohibited.

ID Card. Retiring bargaining unit members shall be issued a departmental identification card, which states “retired,” consistent with Federal Law.
ID Card. The MCP ID card shall contain: i. The MCP’s name as stated in its article of incorporation and any other trade or DBA name used; ii. The name of the member(s) enrolled in the MCP and each member’s medical management information system billing number; iii. The name and telephone numbers of the PCPs assigned to the members; iv. Information on how to obtain the current eligibility status of the members; v. Coordinated Services Program (CSP) information as specified by ODM; vi. Pharmacy information; and vii. The MCP’s emergency procedures including the toll-free call-in system phone numbers.

ID Card. The card is issued to the Insured Person by the TPA to avail Cashless facility in the Network Hospital only. Upon the cancellation or non-renewal of this policy, all ID cards shall immediately be returned to the TPA at the insured’s expense and each Insured Person agrees to hold and keep harmless, the Company and the TPA against any or all costs, expenses, liabilities and claims arising in respect of use or misuse of such ID cards prior to their return to the TPA.

For children and individuals eligible solely through the NJ FamilyCare Program, the identification card must clearly indicate "NJ FamilyCare"; for children and individuals who are participating in NJ FamilyCare Plans C, D, and H the Amended as of September 1, 2003 V -19 in this contract. The contractor shall make provision for continuing all management and administrative services until the transition of enrollees is completed and all other requirements of this contract are satisfied. The contractor shall be responsible for the following: 1. Identification and transition of chronically ill, high risk and hospitalized enrollees, and enrollees in their last four weeks of pregnancy. 2. Transfer of requested medical records.

ID Card. Identity Cards shall be issued by the Insurer/TPA to all the persons covered under the policy. In case of employees, a separate ID card must be issued to each member of the family. If there is a delay in the issuance of ID card by the Insurer/TPA, the ID card issued by the Insured as well as e-cards to its employees, institute fellow and students must be honoured in all the Network hospitals. In case of family members of the Insured employees, any ID card such as Driving License, Voter ID, PAN Card, Passport, Student ID Card, accompanied by the employee’s ID card should be honoured.
