Impaired Physician. The PHT will continue to comply with applicable requirements of the Profes- sionals Resource Network (“PRN”) and to use their services as a primary resource for confidential HSO post-treatment monitoring. Initial confidential evaluations and treatment will be coordinated by the HSOs’ (“HSO”) health insurance plan. The parties agree that nothing in this Article shall be con- strued as a waiver by the Union of any individual HSO’s rights under the Health Insurance Portability and Accountability Act (HIPAA), as amended, or under any other federal, state, or local law. Any HSO on leave from work and using services through the PRN shall provide their program director regular written updates (every 30 days) from the PRN or treatment provider regarding the HSO’s status, including the treatment program the HSO is en- rolled in/receiving treatment through; the anticipated length of the program/ treatment; any deviations from or extensions to the length of the program/ treatment; and the anticipated return to work date of the HSO; provided that the HSO shall not be punished if the failure to provide a timely report is due to delays by the PRN or treatment provider. With the exception of updates regarding anticipated return to work dates, these updates shall be provided to the program director not less than every thirty (30) days while the HSO remains on leave from work. An update re- xxxxxxx a HSO’s anticipated return to work date shall be provided to the program director within five (5) business days of any change thereto. Changes in a HSO’s treatment programs or plans, including, but not limited to, changes to the anticipated length of the program/treatment or anticipat- ed return to work date, shall not be the basis for disciplinary action by the Public Health PHT (“PHT” or the “PHT”). Notwithstanding, absent written approval from the Chief Physician Executive or designee, a HSO’s leave for treatment under this article shall not exceed one (1) year. Should the leave for treatment exceed one (1) year without written approval from the Chief Physician Executive or designee, at that time the HSO will be considered to have voluntarily resigned and withdrawn from the Graduate Medical Ed- ucation program.

  • Physician Visits This plan covers the services of a physician or other provider in charge of your medical care while you are inpatient in a general or specialty hospital.

  • Post Retirement Health Care Benefit Employees who separate from State service and who, at the time of separation are insurance eligible and entitled to immediately receive an annuity under a State retirement program, shall be entitled to a contribution of two hundred fifty dollars ($250) to the Minnesota State Retirement System’s (MSRS) Health Care Savings Plan. Employees who have a HCSP waiver on file shall receive a two hundred fifty dollars ($250) cash payment. If the employee separates due to death, the two hundred fifty dollars ($250) is paid in cash, not to the HCSP. An employee who becomes totally and permanently disabled on or after January 1, 2008, who receives a State disability benefit, and is eligible for a deferred annuity under a State retirement program is also eligible for the two hundred fifty dollar ($250) contribution to the MSRS Health Care Savings Plan. Employees are eligible for this benefit only once.

  • Benefit Level Two Health Care Network Determination Issues regarding the health care networks for the 2017 insurance year shall be negotiated in accordance with the following procedures:

  • School Psychologists At the time of employment and subject to (b) above, full credit for specialized work as a psychologist in a school program shall be given. Full credit for other clinic experience may be given, subject to approval by the Human Resources Division.

  • Patients The Dentist shall accept Covered Persons as patients as reasonably permitted by the Dentist's patient load and appointment calendar. The Dentist will provide Covered Dental Services to Covered Persons on the same basis as to the Dentist's other patients (for example: scheduling, quality of service, and fee charges). The Dentist will be solely responsible to Covered Persons for dental advice and treatment; SDC will have no control over Dentist's practice or the dentist-patient relationship.

  • DRUG/ALCOHOL TESTING 11.1 The parties agree that the maintenance of a drug/alcohol free work place is a goal of both the College and the Union. Employees are prohibited from possession, consumption and/or being under the influence of a controlled substance/alcohol while on the College’s premises or during time paid by the employer. Violations of this prohibition may result in a disciplinary action up to and including termination.

  • Diagnosis For a condition to be considered a covered illness or disorder, copies of laboratory tests results, X-rays, or any other report or result of clinical examinations on which the diagnosis was based, are required as part of the positive diagnosis by a physician.

  • CHILD AND DEPENDENT ADULT/ELDER ABUSE REPORTING CONTRACTOR shall establish a procedure acceptable to ADMINISTRATOR to ensure that all employees, agents, subcontractors, and all other individuals performing services under this Agreement report child abuse or neglect to one of the agencies specified in Penal Code Section 11165.9 and dependent adult or elder abuse as defined in Section 15610.07 of the WIC to one of the agencies specified in WIC Section 15630. CONTRACTOR shall require such employees, agents, subcontractors, and all other individuals performing services under this Agreement to sign a statement acknowledging the child abuse reporting requirements set forth in Sections 11166 and 11166.05 of the Penal Code and the dependent adult and elder abuse reporting requirements, as set forth in Section 15630 of the WIC, and shall comply with the provisions of these code sections, as they now exist or as they may hereafter be amended.

  • Alcohol Testing Alcohol testing will be conducted by using an evidential breath testing device (EBT) approved by the National Highway Traffic Safety Administration. Non-EBT devices may be used for initial screening tests. A screening test will be conducted first. If the result is an alcohol concentration level of less than 0.02, the test is considered a negative test. If the alcohol concentration level is 0.02 or more, a second confirmation test will be conducted. Levels of .04 or greater on the confirmation test will be considered positive.
