Long Term Disability Payments. Disability benefits for the ninetieth (90th) consecutive day following the beginning of the absence to the termination of entitlement to disability benefits shall also be at 60% of the employee's pre- disability earnings as determined at the commencement of the disability. Payment shall be made to the employee monthly during this period by the insurance carrier. The maximum benefit during this period shall be $5,000.00 per month.

Long Term Disability Payments. Disability benefits for the ninetieth (90th) day following the beginning of the absence or upon the exhaustion of all accrued sick leave shall also be sixty (60) percent of the employee’s gross annual base salary as determined at the commencement of the disability divided by twelve (12). Payment shall be made to the employee monthly during this period. The maximum benefit during this period shall be a monthly amount as determined by the insurance carrier. The maximum compensation for both long-term and short-term disability shall be no less than any school district within SAU #29 for the term of this Agreement.
Long Term Disability Payments. It is understood and agreed between the Co-operative and the Union that Long Term Disability payments to entitled employees shall be the responsibility of the Co-operative. If payment of valid claims is not made by the Insurance Company within two (2) weeks from the time the Co-operative receives the completed application, the Co-operative shall then pay to the claiming employee an amount equal to his entitlement. Similarly, when payments are stopped by the Carrier, while the employee's entitlement continues, the employee shall be able to claim the amount of his entitlement from the Co-operative. Payments made by the Co-operative for claims later found to be invalid, or payments made by the Co- operative which are later paid by the Carrier, shall be returnable to the Co-operative. Long Term Disability Payments. (full-time and regular employees): Disability benefits for the ninetieth (90th) day following the beginning of the absence or upon the exhaustion of all accrued sick leave shall also be sixty (60) percent of the employee’s gross annual base salary as determined at the commencement of the disability divided by twelve (12). Payment shall be made to the employee monthly during this period. The maximum benefit during this period shall be a monthly amount as determined by the insurance carrier. The maximum compensation for both long-term and short-term disability shall be no less than any school District within SAU #29 for the term of this Agreement. The Board will continue to contribute toward health and dental insurance as indicated in this policy while an employee is out on long-term disability for a period of eighteen (18) months following the date of initial disability. At the end of this period, subject to the rules and regulations of the health and dental insurers, the employee will be eligible for an additional eighteen (18) months of coverage at his/her own expense under COBRA. It is the responsibility of the Human Resources Office to notify the employee when Board contributions cease.
Long Term Disability Payments. A) An employee on the seniority list, hired prior to September 16, 2018 and has at least 10 years of service, contributions will be made by the Company (based on the last hourly base wage rate, excluding premiums, utilizing a standard 40 hour work week) while on Long Term Disability.

  • Long Term Disability The Employer agrees to provide Long Term Disability benefits for active full-time employees after fifty-two (52) weeks if an Employee is unable to perform any occupation (reasonably suited by means of training, education or experience). The Plan will provide for sixty-six and two thirds percent (66 2/3%) of an Employee's basic monthly earnings to a maximum of $1,500.00. Coverage would cease the date an Employee attains normal retirement age.

The City, at its own cost, shall provide to employees a Long Term Disability (LTD) benefit that provides, after a one hundred and eighty (180) day elimination period, sixty percent salary (60%) (subject to integration) up to age sixty-five (65). Employees who are receiving or who are eligible to receive LTD shall be eligible to participate in the City's Catastrophic Illness Program as set forth in the ordinance governing such program.

  • Long Term Disability Insurance Plan The Employer shall provide a mutually acceptable long-term disability insurance plan, a copy of which shall appear in Appendix “A” – Long-Term Disability Insurance Plan. The plan shall provide post-probationary regular employees with salary continuation as per Appendix “A” until age sixty-five (65) in the event of a disability. The cost of the plan shall be borne by the Employer.
