Management of Navigators. Subrecipient’s care integration coordinator (or in-person Assister supervisor) shall provide management and oversight directly to the Navigators. The coordinator plays an active role in building and sustaining partnerships with community organizations. The Subrecipient uses an asset-based model of care to link clients to economic, social, health, mental health, and community supports via State agencies and community-based organizations. This model consists of building and sustaining relationships with clients to assist them in improving their quality of life and health.

Management of Navigators. The Subrecipient shall provide 10 Navigators to perform the services outlined within this agreement. The total staff hours per week dedicated to serving as Navigators shall be 96, and collectively will equal 2.25 FTE. These employees shall be supervised by the Outreach Enrollment Coordinator.
Management of Navigators. The Subrecipient will staff 15 navigators over the course of this agreement who will be supervised by local supervisors and report through Office Managers and ultimately to the Executive Director. The total combined navigator hours per week are: 78.4 through 9/30/14; 218.4 through 12/31/14; 90.4 through 3/31/15; and 56.4 through 6/30/15.

Management of Navigators. The Subrecipient will maintain one full-time employee to provide the services outlined under this agreement, at forty (40) service hours per week. The supervision of the program will be a collaboration of the Blueprint for Health team and Patient Access and Financial Service Team. The director of the supervising department will ensure that the Navigator is submitting reports on time and maintains compliancy with the grant.

The Subrecipient shall ensure that all staff assigned to this project to provide these services shall attends all state required trainings and pass the State certification process. The Direct Services Manager will directly supervise Navigators. Additional interpreters will aid Navigators in communication with enrollees but will not complete applications on their own. Management of Navigators. The Navigator Program will be overseen by the Program Manager/Coordinator (0.5 FTE). The Grantee will hire 1 FTE “Roving” Navigator who will be responsible for traveling the state, making appointments with businesses, conducting informational forums and, eventually, enrollment workshops, and working with all of our partners in coordinating outreach efforts. The Roving Navigator will also reach out to the business community in areas where there is no partner organization to directly to schedule appointments and events. The Roving Navigator will have a presence in each county several times per month throughout the summer and fall. In addition, there will be two full-time “in office Enrollment Specialists” who will provide enrollment assistance at the Grantee’s main office. The Program Manager/Coordinator will also oversee the Navigator work at the eight identified partner organizations, collect monthly reports from them and submit aggregate reports to the state per the reporting requirements outlined below.
