Management of Navigators. Subrecipient’s care integration coordinator (or in-person Assister supervisor) shall provide management and oversight directly to the Navigators. The coordinator plays an active role in building and sustaining partnerships with community organizations. The Subrecipient uses an asset-based model of care to link clients to economic, social, health, mental health, and community supports via State agencies and community-based organizations. This model consists of building and sustaining relationships with clients to assist them in improving their quality of life and health.
Management of Navigators. The Subrecipient shall staff 2 FTE Navigators to fulfill the services outlined in this agreement. The Subrecipient determines 1 FTE as performing 37.5 hours of service per work. Combined, the Navigators shall provide a total of 75 service hours per week. All Navigators will be directly supervised by the Lead Navigator.
Management of Navigators. The Grantee’s Executive Director will provide leadership and administrative oversight for the group. The Grantee will hire two certified contract-based Navigators. The Executive Director will provide direct supervision of the Navigators, including managing the grant funding and compensation for the two contract Navigators. As the Navigator certification criteria dictates, individuals designated to become Navigators by Grantee must complete the Navigator training program and pass an exam to demonstrate an understanding of the information taught in the training program. This is a two-step process:
Management of Navigators. The Subrecipient will provide 2 current staff to be trained as Navigators and, if needed, will add 1 new position as backup. The total weekly staff hours for the 2 current staff will equal 58, and if backup Navigator is needed they will provide up to 24 hours per week. The Subrecipient will hire 1 part-time administrative staff person and will purchase a software program with a time and contact tracking component in order to accurately monitor activities and Navigator hours worked.
Management of Navigators. The total number of full-time employees (FTE) to be dedicated to the provision of Navigator services will be 1 FTE, which will be provided by one individual. In addition, the organization shall plan to train 1 additional individual to become a Certified Application Counselor (CAC). The Navigator position shall be supervised by the Director of Community Initiatives/Blueprint Project Manager. Total staff hours per week dedicated to serving as a Navigator shall be forty (40).
Management of Navigators. The Subrecipient will provide one (1) FTE Navigator, which will be split between four community health workers, to fulfill the services outlined in this agreement. The Navigators shall provide forty (40) service hours per week and will be managed by the Care Integration Coordinator.
Management of Navigators. The Subrecipient will hire 1 full-time staff to be certified as a Navigator. Their total hours per week will be 40. The Subrecipient shall ensure that all staff assigned to this project to provide these services shall attends all state required trainings and pass the State certification process. It is the Subrecipient’s responsibilities to obtain all policies and procedures for Navigators, ensuring that all compliance requirements of the grant are met. As the Navigator certification criteria dictates, individuals designated to become Navigators by Subrecipient must complete the Navigator training program and pass an exam to demonstrate an understanding of the information taught in the training program. Management of Navigators. The Subrecipient is working in conjunction with the Lamoille Region Chamber of Commerce (LRCC) to perform the duties outlined in this agreement. The Subrecipient shall provide 1.3 full time employees (FTE). An FTE is defined as performing forty (40) service hours per week. The Navigators shall be managed by the Directors of the Subrecipient’s organization and LRCC.
Management of Navigators. The Grantee’s Executive Director will be responsible for the overall project management of the grant. In addition, each Clinic Director will be responsible for the onsite management of their specific Navigator program. The Subrecipient shall provide 5-8 FTEs including both employees and certified Navigator volunteer.
Management of Navigators. The Subrecipient will staff 15 navigators over the course of this agreement who will be supervised by local supervisors and report through Office Managers and ultimately to the Executive Director. The total combined navigator hours per week are: 78.4 through 9/30/14; 218.4 through 12/31/14; 90.4 through 3/31/15; and 56.4 through 6/30/15.
