Measures and Targets. The program will include a combination of HEDIS and other measures, as finalized by the Department and as described in the HMO P4P Guide for MY2015. Unless otherwise specified, performance targets for each measure will be of two types – Level and Degree of Improvement. Level targets are designed to recognize and give credit to HMOs with already high performance. Degree of Improvement targets are designed to recognize HMOs that are substantially below the Level targets but are making significant improvements in their performance. NCQA’s Quality Compass results will be used to set the Level targets for HEDIS measures, and other Statewide results will be used for non-HEDIS measures. The Degree of Improvement targets will be set using the past performance of each HMO (i.e., baselines, when available), or state- wide averages. Further details of the methodology for setting targets are specified in the Guide.

Measures and Targets a. Initial Measures Initial measures represent activities happening when children are enrolled in County PIHP. These include: i. Childhood Behavior Checklist The Child Behavior Checklist tool, with a minimum assessment frequency of member intake and discharge, until a new tool is determined. The County must continue to collect and report outcome indicator information until the new tool is indicated.
Measures and Targets. The Threshold, Mid-Point and Maximum measures and targets as approved by the Committee for Section 16 Officers under the Annual Incentive Plan (“AIP”) for FY2010, FY2011, and FY2012 shall be the Threshold, Mid-Point and Maximum measures and targets for this Award for FY2010, FY2011, and FY2012, respectively. The applicable achievement percentage shall be interpolated in relation to the foregoing; provided, however, that if any measure achieved is less than the Threshold amount, the achievement percentage for that measure shall be zero; and, provided, further, that in no event shall the achievement percentage exceed 200%. Except to the extent provided in Paragraphs 5 through 7 below, the amount of the Award earned under this Agreement and under any other Performance Cash Awards granted to you under which amounts may be earned as a result of the Company’s performance during the Performance Period shall be determined after the end of the Performance Period. The achievement percentage of each measure shall be equal to the average of the actual achievement percentages for each year of the Performance Period for which such Awards have been issued multiplied by the sum of the Mid-Point Award values for the three-year Performance Period. The three-year average achievement percentage for each measure is weighted and summed to determine the amount of the award earned. For this purpose, the measures for this Award will be weighted in the same relative proportion as under the AIP.

  • Milestones Subject to the provisions of the SGIP, the Parties shall agree on milestones for which each Party is responsible and list them in Attachment 4 of this Agreement. A Party’s obligations under this provision may be extended by agreement. If a Party anticipates that it will be unable to meet a milestone for any reason other than a Force Majeure event, it shall immediately notify the other Parties of the reason(s) for not meeting the milestone and (1) propose the earliest reasonable alternate date by which it can attain this and future milestones, and (2) requesting appropriate amendments to Attachment 4. The Party affected by the failure to meet a milestone shall not unreasonably withhold agreement to such an amendment unless it will suffer significant uncompensated economic or operational harm from the delay, (1) attainment of the same milestone has previously been delayed, or (2) it has reason to believe that the delay in meeting the milestone is intentional or unwarranted notwithstanding the circumstances explained by the Party proposing the amendment.

  • Interim Measures Notwithstanding any requirements for alternative dispute resolution procedures as set forth in Articles 18(B), any party to the Dispute may apply to a court for interim measures (i) prior to the constitution of the arbitral tribunal (and thereafter as necessary to enforce the arbitral tribunal’s rulings); or (ii) in the absence of the jurisdiction of the arbitral tribunal to rule on interim measures in a given jurisdiction. Hearings on requests for interim measures may be held in person, by telephone, by video conference or by other means that permit the parties to the Dispute to present evidence and arguments.
