Medical and Other Insurance Benefits. ● Employee may elect COBRA continuation coverage under the Company group health plan, to the extent permitted under the applicable plan terms and COBRA. Any conversion or other rights shall be governed by the terms of the applicable plans.

Medical and Other Insurance Benefits. The City makes available to eligible Employees a number of medical, dental and life insurance benefit programs. For Employees in Classes represented by a Recognized Employee Organization, the City and/or Employee share of the premium costs are determined through the meet and confer process and are included in the written memorandum of understanding. For eligible Employees who are not members of a Recognized Employee Organization, the City’s share and the Employee’s share of the premium costs shall be determined by the City Manager at the point of hire and annually thereafter based on the City's fiscal capability. All Regular Employees and Executive Management Employees are eligible to participate in health and life insurance programs provided by the City effective with the Employee's Service Date.
Medical and Other Insurance Benefits. The Superintendent of Schools shall be afforded comprehensive insurance for herself and her family as she elects according to the terms of insurance policies available to employees in the District, including hospitalization, major medical, dental insurance and vision coverage. During the term of this Agreement the Board shall also provide the Superintendent with term life insurance in an amount equal to Four Hundred Thousand Dollars ($400,000.00), the ownership of which, if allowed by the issuing insurance company, may be transferred to the Superintendent at her expense upon termination of this Agreement for any reason.

Because the qualifying event -- Meteny's termination -- occurred on August 19, 1999, Meteny acknowledges that if these benefits continue in effect for eighteen (18) months or more, Meteny will not be entitled to COBRA coverage.

  • Other Insurances Contractor may be required to carry additional insurance based upon the nature of the work to be performed (scope of services). For each additional required insurance, a corresponding certificate of insurance must be provided.   • Other Insurance If requested by the Director, Contractor shall furnish adequate evidence of Social Security and Unemployment Compensation Insurance, to the extent applicable to Contractor’s operations under this Agreement.

  • Errors and Omissions, Professional Liability or Malpractice Insurance Contractor may be required to carry errors and omissions, professional liability or malpractice insurance. All policies shall remain in force through the life of this Contract and shall be payable on a "per occurrence" basis unless County specifically consents to a "claims made" basis. The insurer shall supply County adequate proof of insurance and/or a certificate of insurance evidencing coverages and limits prior to commencement of work. Should any of the required insurance policies in this Contract be cancelled or non-renewed, it is the Contractor’s duty to notify the County immediately upon receipt of the notice of cancellation or non-renewal. If Contractor does not carry a required insurance coverage and/or does not meet the required limits, the coverage limits and deductibles shall be set forth on a waiver, Exhibit C, attached hereto. Failure to provide and maintain the insurance required by this Contract will constitute a material breach of this Contract. In addition to any other available remedies, County may suspend payment to the Contractor for any services provided during any time that insurance was not in effect and until such time as the Contractor provides adequate evidence that Contractor has obtained the required coverage.

  • Life Insurance Benefits A. During the life of this Agreement, the basic life insurance benefit made available to Faculty members shall be calculated as 3 times base annual earnings, rounded to the next highest $1,000, but not more than $225,000. A separate additional benefit up to the amount of the life insurance will be paid for accidental death and dismemberment, or loss of sight. The amount of Life and Accidental Death and Dismemberment/Loss of Sight benefits will be reduced to 65% at age 65, and further reduced (from the original insurance amount) as follows: to 50% at age 70, and 35% at age 75. Basic life insurance and AD&D benefits will be provided with no employee contributions. B. Faculty members will be eligible to purchase the following supplemental coverage: 1. additional amounts of group term life insurance at a level of between one and three (3) times the Faculty member’s annual salary with a maximum of $600,000. The guaranteed issue level at initial enrollment will be determined by the life insurance carrier and any amounts over the guaranteed level will be subject to the underwriting requirements of the life insurance carrier. 2. group term life insurance for spouses and domestic partners at a level of between one (1) and three (3) times annual salary with a maximum of $600,000.   • Deductibles and Self-Insurance Retentions Any deductibles or self-insured retentions must be declared to and approved by the City. The City may require the Consultant to provide proof of ability to pay losses and related investigation, claims administration and defense expenses within the deductible or self-insured retention. The deductible or self-insured retention may be satisfied by either the named insured or the City.
