Membership in the Catastrophic Leave Bank. 10.22.3.1. The unit member who wishes to participate in the Catastrophic Leave Bank must donate one (1) full salary sick leave day or one (1) full salary vacation day to the bank during the designated donation period(s) which shall occur annually during the month of May. Following the initial donation period, additional donations shall be requested only when the balance in the bank drops below two-hundred (200) days. However, nothing herein shall preclude any eligible unit member from donating to the bank at any time.

Membership in the Catastrophic Leave Bank. Any unit member or 11 certificated manager who has been employed by the District for at least 12 eleven (11) calendar months is eligible for membership in the Catastrophic 13 Leave Bank. Membership begins when sick leave is donated.
Membership in the Catastrophic Leave Bank. Any bargaining unit member who has been employed by the District for at least 30 days is eligible for membership in the Catastrophic Leave Bank. 10.12.2.1 The District will conduct an "enrollment period" after the creation of the leave bank for sixty calendar days. All current District bargaining unit members who wish to be a member of the Catastrophic Leave Bank must contribute one day of leave into the Catastrophic Leave Bank at this time. Bargaining Unit members hired after the initial enrollment period, or members that are returning from extended leave, which included the initial enrollment period, will have sixty calendar days to join the Catastrophic Leave Bank by contributing one day of leave into the bank. After the initial enrollment period, members of the Catastrophic Leave Bank must contribute one day of leave at the beginning of each school year if the leave bank falls below 2,400 hours during the preceding school year. All donations will be in whole-day increments, with a maximum of one day of leave being donated.

  • Catastrophic Leave Bank The City agrees to establish a Catastrophic Leave Bank to assist employees who have exhausted accrued leave time due to a serious or catastrophic illness or injury. The Catastrophic Leave Bank (CLB) will allow the bargaining unit employees to donate time to affected employees within and outside the unit, so that he/she can remain in a paid status for a longer period of time, thus partially ameliorating the financial impact of the illness, injury or condition. This donated time will be placed in a CLB and drawn down from the CLB by the eligible employee. Eligibility To be eligible for this benefit, the receiving employee must: 1) Be a regular full time employee, 2) Have sustained or have an immediate family member who has sustained a life threatening or debilitating illness, injury or condition which may require confirmation by a physician, 3) Have exhausted all accumulated paid leave including vacation, holiday, sick leave, and/or compensatory time off, 4) Be unable to return to work for at least 30 days or in the case of the condition affecting the immediate family member, that member must be in need of prolonged and significant personal care; and 5) Conformed with the requirements of the Family Medical Leave Act and/or Worker's Compensation.

  • Catastrophic Leave The County will administer a Catastrophic Leave procedure designed to permit individual donations of annual leave, vacation, healthcare leave (8 hours maximum per fiscal year), compensatory and/or PIP leave time to an employee who is required to be on an extended unpaid leave due to a catastrophic medical condition or other serious circumstances.

For any other error or question you have involving the billing statement of the participating merchant, you must contact the merchant directly. We are not responsible for investigating such errors. - Any other exceptions as established by the Credit Union.

  • Coverage Under the Minnesota Advantage Health Plan From July 1, 2019 through December 31, 2019, health coverage under the SEGIP will continue at the level in effect on June 30, 2019.
