Monthly Performance Report. Beginning January 1, 20167, Contractor shall monitor and track its performance each month against the Performance Measurement Standards set forth in Group 1 and 2. Contractor shall provide detailed supporting information (as mutually agreed by the parties) for each Monthly Performance Report to the Exchange in electronic format. Contractor shall report Exchange business only and shall report Contractor’s Exchange Enrollees in the Individual Exchange separate from Contractor’s Exchange Enrollees in Covered California for Small Business.

Monthly Performance Report. The NAVITAIRE Account Manager will submit a Monthly Performance Report by the sixth business day of the subsequent month following the Reporting Period to the Customer Account Liaison. The report will contain the monthly indicator of Service Levels statistics and will be transmitted via email unless otherwise requested by the Customer. The report will also summarize all Interrupted Service Reports for the Reporting Period.

Monthly Performance Report. SUB-RECIPIENT shall submit a Monthly Report no later than by the tenth day (10th) each month for the previous month. The report shall include, but not be limited to: enrollments, entered and completed work-based learning/training, how many attained certificate, entered and completed OJT, entered employment, training-related employment, exits, partnerships developed, supportive services provided, and average wage for the employment placements.

Monthly Performance Report. Each calendar month during the Term of this Agreement, Servicer will deliver to Buyer Agent a monthly summary report, in computer file form reasonably accessible and usable by Buyer Agent, or via access to an electronic portal containing such information, as of the end of the immediately preceding calendar month, with respect to all Allocated Eligible Options, which shall include (a) a monthly collections report (in form and with details and reporting information reasonably acceptable to Buyer Agent) on the entire portfolio of Allocated Eligible Options, (b) Option Level Detail regarding the entire portfolio of Allocated Eligible Options, and (c) such other details as Buyer Agent and Servicer may from time to time reasonably agree, all prepared by Servicer and certified to its knowledge as being true, correct and complete in all material respects as reported to and understood by the Servicer. Such summary report and certification of the Servicer shall be delivered to Buyer Agent on or before the tenth (10th) Business Day after the end of each month.
