Personnel Documentation. Employment applications shall include results of Department of Motor Vehicles checks, verification of employment, reference checks, criminal history verifications, credit checks, drug screening, physical examinations, language proficiency, and personality assessments will be made available to the GS/OAS prior to entering GS/OAS property. The documents described above are to be maintained in the files of the Contractor, and provided to the GS/OAS upon request. The Contractor will certify in writing that all employment prerequisites have been fully met prior to Contractor employees accessing any GS/OAS property.

Personnel Documentation. When required by CCH, Vendor will provide documentation evidencing that all personnel assigned by Vendor to furnish Services or Deliverables on-site at a CCH or other County facility will have had a fingerprint-based background check, drug and TB tests, and designated immunizations and vaccinations. Vendor will bear the costs of all such checks, tests and immunizations and vaccinations.
Personnel Documentation. At District’s request, Client will provide District with: (i) the names of every Client employee and or other persons who will be providing Services; (ii) evidence of the licensure, certifications, training and other qualifications of every Client employee or other person providing Services; (iii) evidence of the fingerprinting required by Section 3.3; and (iv) evidence of the immunizations and testing required by Section 3.4.
Personnel Documentation. Teachers will have the right upon written request to review the contents of their personnel file. A teacher will be entitled to have a representative of the Association present when reviewing the contents of his/her personnel file. No material derogatory to a teacher’s conduct, service, character or personality arising during the course of his/her employment, or any other material, will be placed in this personnel file unless the teacher has had an opportunity to review such material. The teacher will acknowledge that he/she has had the opportunity to review such material by affixing his/her signature to the copy to be filed with the express understanding that such signature in no way indicates agreement with the contents thereof. The teacher will also have the right to submit a written answer to such material, and his/her answer shall be reviewed by the Superintendent and attached to the file copy.
