Wellness Plan. The District will provide a Wellness Program designed to improve the health of the District employees and that will result in both short-term and long-term projected savings in health insurance costs. Employees that fully participate in Paladina or the Wellness Program will have their share of premium capped at eight percent (8%) effective July 1, 2020. Other employees who do not participate in the Wellness Program or Paladina will contribute thirteen percent (13%) effective July 1, 2020.
Wellness Plan. Your Physician will provide you with a personalized wellness plan, to be updated at your annual visits, focusing on appropriate screening and early detection of medical problems leading to early diagnosis and treatment. The preparation of the wellness plan is at no additional cost to you; but you will be financially responsible for those services outlined in your wellness plan.
Wellness Plan. 7.5.3.4.1 In order to advance the goals of strengthening preventive services and providing integrated physical, behavioral health, and dental services to all Eligible Persons, the Contractor shall develop a Wellness Plan. 7.5.3.4.2 The Wellness Plan shall include a strategy for coordination with government agencies of the Government of Puerto Rico integral to disease prevention efforts, including the Puerto Rico Health Department, the Department of the Family, and the Department of Education. 7.5.3.4.3 The Wellness Plan shall present strategies for encouraging Enrollees to: 7.5.3.4.3.1 Seek an annual health checkup; 7.5.3.4.3.2 Appropriately use the services of MiSalud, including Tele MiSalud; 7.5.3.4.3.3 Seek women’s health screenings including mammograms, Pap smears, cervical screenings, and tests for sexually transmitted diseases; 7.5.3.4.3.4 Maintain a healthy body weight, through good nutrition and exercise; 7.5.3.4.3.5 Seek an annual dental exam; and 7.5.3.4.3.6 Attend to the medical and developmental needs of children and adolescents, including vaccinations. 7.5.3.4.4 The Contractor shall, according to the timeframe specified in Attachment 12 to this Contract, present its Wellness Plan containing the strategies described above, to ASES for review and approval, which approval will not be unreasonably withheld, conditioned or delayed. Any subsequent changes to the Wellness Plan must be previously approved in writing by ASES.
Wellness Plan i. A wellness program will be put into place January 1, 2011. ii. The plan design is to be determined by the CPS Benefit Committee. iii. The plan will cover Disease Management, Lifestyle Management and Case Management. iv. Employees & spouses will be provided with the opportunity to earn $500 each per year in wellness credit. Xxxxxx credits will be applied to individual Health Reimbursement Accounts (HRA) in the calendar year following the year in which they were earned. (2011 credits will be applied in 2012) v. Maximum HRA balances will be $1,500 at any one time (applies to both Single/Family accounts) vi. Employees will have 6 months to submit HRA claims incurred prior to their separation from employment. Any balances remaining in the HRA after this 6 month period will be returned to CPS.
Wellness Plan. Effective January 1, 2007, a Wellness Plan shall be established to invest in prevention through measurement, education and reward. Employee contributions when enrolled in the health insurance plan are indexed to participation in the City’s wellness plan. Activities and events in the wellness plan may include but are not be limited to: Health Risk Assessment (HRA) Biometric and Blood Pressure Screenings Wellness Fair Health Related Education and Fitness Events Smoking Cessation The City may also be entitled to offer additional rewards through offerings unique to the HSA Plan participants. The Health Focus Group shall explore expansion of the Wellness Plan, subject to the City’s approval of any cost issues. The group will also serve as a communication channel on plan changes and implementation.
Wellness Plan. WCB agrees to fund a Union Administered Wellness Plan at the rate of 1.6% of regular employee gross salary. The Union will provide a qualified statement annually to the Employer by March 31 of each year, with a non qualified statement by December 31 of the final year of the collective agreement. Upon request from the Employer the Union will provide information as to the nature of plans or programs these funds are allocated to. The WCB will not be responsible for any cost overruns which may occur in the allocation of funds. The Employer and the employees will continue to participate in and contribute at the rate of 7.25% each to the Public Employees Pension Plan (PEPP). Effective January 1, 2022 the Employer and the employees will contribute at the rate of 7.75% each to the Public Employees Pension Plan.

The wellness program shall include the following: a. Health risk appraisal or assessment Early detection programs will include; blood pressure, basic blood diagnostics, and cholesterol and glucose tests. b. The employer shall provide to employees and their spouses the following optional elements: Behavior and lifestyle counseling including nutrition, alcohol, and tobacco.
Wellness Plan. 33.1.1 WCB agrees to fund a Union Administered Wellness Plan at the rate of 1.6% of regular employee gross salary. 33.1.2 The Union will provide an audited statement annually to the Employer by March 31 of each year. 33.1.3 The SGEU will provide ongoing reporting as to the nature of plans or programs these funds are allocated to. 33.1.4 The WCB will not be responsible for any cost overruns which may occur in the allocation of funds.

Annual voluntary physical for the employee and employee’s spouse, at a physician of the employee’s and employee’s spouse’s choice, with cost fully paid by the City; 2. Annual voluntary flu shot for the employee and employee’s spouse, with cost fully paid by the City; 3. A reduced employee cost voluntary fitness club membership covering the employee and the employee’s dependents;
