Dental and Vision Insurance. The Agency shall continue dental and vision plans that permit dependent coverage. The Agency shall continue to pay the premium for dental and vision coverage for the employee only. Dependent coverage shall be paid by the employee.

Dental and Vision Insurance. The City will provide dental and vision insurance to employees and eligible dependents according to the terms of its insurance plans. The City will not make changes to its dental or vision insurance plans during the term of this Agreement without first bargaining with the Joint Labor Committee. The City will pay the full premium cost for dental and vision insurance for employees and eligible dependents. Part time employees working at least twenty (20), but less than thirty (30) hours per week will be responsible for a prorated contribution toward the cost of the plan based on the percentage that the part-time employee’s FTE actual hours compensated in the previous month bears to full-time (40 hours per week). Part-time employees working thirty (30) or more hours per week will make contributions equal to those of full time employees. For all other purposes or benefit calculations, the City’s definitions and policies regarding part-time employment will govern.
Dental and Vision Insurance. The District will maintain its current dental and vision insurance plans for employees. The District will pay the monthly premium for each eligible employee.

Dental and Vision Insurance. A Preferred Provider (PPO) dental plan will be provided which covers one hundred percent (100%) of the aggregate costs of preventive and diagnostic services, maintenance dentistry, and restorative dentistry provided by a PPO participant. Non-PPO provided services will be covered at a reduced rate of eighty percent (80%). Such coverage will include dental services ranging from examinations, cleaning and fillings to caps, crowns, bridges and root canals. Orthodontic services will not be provided. The Board will contribute eighty-five percent (85%) toward the cost of single dental coverage. For faculty members opting for family dental coverage, the Board will contribute seventy-five percent (75%) toward the cost of family, employee/children or employee/spouse dental coverage. A faculty member must be enrolled in the group medical plan to be eligible for dental plan coverage. A voluntary vision insurance program shall be provided. The Board shall contribute fifty percent (50%) toward the cost of single vision coverage for the plan option selected by the faculty member.
Dental and Vision Insurance. Section 1. The district will provide vision and dental insurance equivalent to what the employees received in 2009-2010. Section 2. If an employee is absent because of illness or off the job injury and notifies the employer of the illness or injury, the employer shall continue to make the required contributions for a period of (4) weeks. If an employee is injured on the job, the employer shall continue to pay the required contribution for a period not to exceed (12) months from the date of the injury. Section 3. If an employee is granted a leave of absence, the Employer shall collect from said employee, prior to the leave of absence, sufficient funds to pay the required contribution. Section 4. Notwithstanding, anything herein contained, it is agreed that in the event any Employer is delinquent at the end of a monthly period in the payment of the contributions to the welfare fund, in accordance with the rules and regulations of the trustees of such fund and after a seventy-two (72) hour notice has been delivered to the Employer by a local Union official, the Union shall have the right to take such action as it deems necessary to collect such delinquent payments, and it is further agreed that the Employer shall be responsible for the losses of the employees resulting thereof. Section 5. By the execution of this Agreement, the employer authorizes the employers associations who are signatories to collective bargaining agreements with the Teamsters unions containing similar provisions, to enter into appropriate trust agreements necessary for the administration of such fund and to designate the employer trustee under such trust agreements, hereby waving all notice thereof and ratifying all actions already taken or to be taken by such trustees within the scope of their authority.

Dental and Vision Insurance. The cost of single coverage for dental and vision insurance will be paid entirely by the Board of Education. Any additional costs for family coverage for dental and vision insurance will be paid entirely by the employee.
