Product Participation. Participating Providers, Preferred Providers, and Government Sponsored Programs Providers participate in all Blue Shield and Health Plan Products utilizing the Participating Provider network, the Preferred Provider network, and/or the Government Sponsored Programs network(s) respectively, and as designated by Blue Shield and/or Health Plan as applicable and so long as Provider and Practitioners, as applicable, meet all required Participation Criteria applicable to a Product by Blue Shield at Blue Shield’s discretion. Provider’s and each Practitioner’s participation in a Product will be subject to all terms and conditions contained in the Agreement and, as applicable, and any related attachments thereto. Provider and Blue Shield and/or Health Plan, as applicable, agree that such participation is not exclusive and that other Providers and Practitioners may also participate as designated by Network Plan. Provider and Practitioners agree that Blue Shield and/or Health Plan, as applicable, in its sole discretion: (a) retains the exclusive authority to designate those providers that may participate in any particular Product and that all such providers will be subject to all applicable participation criteria related to such Product; (b) may choose to require execution of different participation agreements for any particular Product in its sole discretion; (c) may assign providers to a specific tier and/or differential cost sharing network, including Products of other health plans to which Blue Shield has granted access to the Participating Provider network, the Preferred Provider network, and/or the Government Sponsored Programs network(s) through a Network Access Arrangement and (d) provider may be designated to participate in a Product which may utilize multiple professional networks in order to achieve Member access to Provider Services as necessary.
Product Participation. Physician agrees to participate in at least one Product through CCPA.
Product Participation. As of the Effective Date, Provider agrees to participate in the Products designated on the Signature Sheet to this Agreement. Company reserves the right to introduce and designate Provider’s (and/or any specific Group Provider(s)’) participation in new Specialty Programs or products (including, but not limited to, Medicare and/or other government program products) during the term of this Agreement and will provide Provider with at least ninety (90) days advance written notice of such new Specialty Programs/products, along with associated terms and rates and, if applicable, new or amended Product Addendum(a) that describe the applicable Specialty Program/products. Provider shall have thirty (30) days from receipt of Company’s notice to notify Company in writing that Provider elects not to participate in a new Specialty Program/product; provided, however, that a variation of a current Specialty Program or Product at then- current rates and terms shall not be considered to be a new Specialty Program or product under this paragraph. Provider’s failure to provide such notice shall automatically be deemed to constitute Provider’s acceptance of the new Specialty Program or product and associated terms and rates. Nothing in this Agreement shall require that Company identify, designate or include Provider and/or Group Providers as a participant or preferred participant in any specific Specialty Program, Product (or Product variation), generally, or for any specific Payers/customer(s); provided, however, that Provider shall accept compensation in accordance with the applicable Product Addendum and Service and Rate Schedule for the provision of any Covered Services to Members under a Specialty Program or Product. Company may sell, lease, transfer or otherwise convey to third parties the benefits of this Agreement, including, but not limited to, the applicable Service and Rate Schedule(s).
Product Participation. Facility agrees to participate in the Plans and other health benefit programs listed on the Program Participation Schedule., To the extent that Company establishes and/or participates in a provider Pay- for-Performance incentive program or Performance Improvement Programs, Facility agrees to comply with and participate in such program. Nothing herein shall require that Company identify, designate or include Facility as a preferred participant in any specific Plan for which Company provides incentives based upon the use of selected participating facilities, Specialty Program or other program; provided, however, Facility shall accept compensation in accordance with this Agreement for the provision of any Covered Services to Members under a Plan, Specialty Program or other program in which Facility has agreed to participate hereunder.

Company reserves the right to introduce and designate Facility’s participation in new Specialty Programs and products during the term of this Agreement and will provide Facility with written notice of such new Specialty Programs and products and the associated compensation. Nothing in this Agreement shall require that Company identify, designate or include Facility as a preferred participant in any specific Specialty Program or product; provided, however, Facility shall accept compensation in accordance with this Agreement for the provision of any Covered Services to Members under a Specialty Program or product in which Facility has agreed to participate in this Agreement. Company may sell, lease, transfer or otherwise convey to payers (other than Plan Sponsors) which do not compete with Company's product offerings in the geographic area where Facility provides Covered Services, the benefits of this Agreement, including, without limitation, the Services and Compensation Schedule attached hereto, under terms and conditions which will be communicated to Facility in each such case. For those programs and products which are not health benefit products, Facility shall have thirty (30) days from receipt of the Company’s notice to notify Company in writing if Facility elects not to participate in these product(s).
Product Participation. Group and Participating Group Providers agree to participate in the Plans and other health benefit products as described in the Product Participation Schedule. Company reserves the right upon ninety (90) days prior WhiteGlove House Call Health, Inc. notice, to introduce, modify and designate Groups’ and Participating Group Providers’ participation in new Plans, Specialty Programs and products during the term of this Agreement and will provide Group with written notice of such Plans, Specialty Programs and products and the associated compensation. Nothing in this Agreement shall require that Company identify, designate or include Group and Participating Group Providers as a preferred participant in any specific Plan, Specialty Program or product; provided, however, Group and Participating Group Providers shall accept compensation in accordance with this Agreement for the provision of any Covered Services to Members under a Plan, Specialty Program or product in which Group and Participating Group Providers have agreed to participate in this Agreement. Company may sell, lease, transfer or otherwise convey to payers (other than Plan Sponsors) which do not compete with Company’s product offerings (e.g., workers’ compensation or automobile insurers) in the geographic area where Group provides Covered Services, the benefits of this Agreement, including, without limitation, the Services and Compensation Schedule, under terms and conditions which will be communicated to Group in each such case. For those programs and products which are not health benefit products (e.g., worker’s compensation or auto insurance), Group shall have thirty (30) days from receipt of the Company’s notice to notify Company in writing if Group elects not to participate in these products(s).
Product Participation. Hospital agrees to participate in the Plans and other health benefit programs listed on the Program Participation Schedule. Company reserves the right to introduce and designate Hospital’s participation in new Plans, Specialty Programs and other programs during the term of this Agreement and will provide Hospital with written notice of such new Plans, Specialty Programs and other programs and the associated compensation. To the extent that Company establishes and/or participates in a provider Pay-for-Performance incentive program or Performance Improvement Programs, Hospital agrees to comply with and participate in such program. Nothing herein shall require that Company identify, designate or include Hospital as a preferred participant in any specific Plan for which Company provides incentives based upon the use of selected Participating Hospitals, Specialty Program or other program; provided, however, Hospital shall accept compensation in accordance with this Agreement for the provision of any Covered Services to Members under a Plan, Specialty Program or other program in which Hospital has agreed to participate hereunder.

Product Participation. The third paragraph of Section 2.11 Product Participation, shall be deleted and replaced with the following: “Company may sell, lease, transfer information or otherwise convey to third parties the benefits of this Agreement, including, but not limited to, the applicable Service and Rate Schedule(s). Company may contract with a third party entity to provide access to the Company's rights and responsibilities under this Agreement. On the request of Provider Company will provide information necessary to determine whether a particular third party has been authorized to access Provider’s health care services and contractual discounts.”
