Subrecipient shall provide temporary shelter, intake assessment, and case management services to individuals to secure permanent housing, increased income, and connections to public benefits and health services as set forth in 24 CFR 576.102. Such connections, as a result of the intake assessment and dependent upon the needs of the individual, may include, but are not limited to:
Program Essential Requirements. The Program shall meet the County’s need to provide emergency housing intervention services identified in this Contract to those experiencing homelessness through connection of services and an emergency shelter program.
1. Contractor shall provide regional services to victims of crime or other trauma experiencing homelessness in Orange County.
2. Contractor shall provide temporary shelter and case management and advocacy services to victims of crime or other trauma to secure permanent housing, increased income, and connections to public benefits and health services. These include:
a. Weekly individual and/or group counseling
b. Weekly house meetings c. Crisis intervention d. 24-hour coverage to provide round-the-clock support to participants and ensure safety of participants e. Referrals to proximate 12-step meetings to help participants maintain sobriety
Program Essential Requirements. Contractor shall:
1. Ensure that the Program will be in operation Monday - Friday (8 hours a day). In approval from COUNTY.
2. Engage in street outreach, including face-to-face, phone contact, and email contact as appropriate.
3. Provide coordination of regional navigation support for Central Service Planning Area by:
a. Provide service navigation to Participants in the region.
b. Assist County in responding to homeless encampments and homelessness activity hotspots.
c. Providing a clear process for participants and community members seeking to support Participants in accessing street outreach, that includes expected timeframes for response.
4. Provide progressive engagement to individuals experiencing chronic homelessness who are challenging to serve and reluctant to seek assistance due to behavioral health challenges, substance use and chronic health conditions.
5. Connect eligible participants to the appropriate Level of Care, including aid in triaging participants to services that best meet their needs.
6. Follow up with eligible participants to ensure receipt of the appropriate Level of Care and support positive outcomes, including:
a. Increase street outreach discharge destinations to emergency shelter or other sheltered destination
b. Increase housing stability for all participants c. Reduce returns to homelessness
7. Provide referrals that meet the eligible participants’ medical, behavioral and other health needs and assist the eligible participants in obtaining the necessary services.
8. Provide case management services to eligible participants engaged through outreach services.
9. Provide case management to divert participants from shelter through family reunification and other means.
10. Maintain a case management ratio of one Case Manager to 25 participants (1:25) for Program quality of service, positive outcomes and program efficacy.
11. Coordinate with County agencies engaged with those experiencing homelessness volunteers to assist with program services.
Program Essential Requirements. The Program shall meet the County’s need to provide rental assistance and housing relocation and stabilization services identified in this Contract to those experiencing homelessness through evaluation of needs and connection to resources.
1. Contractor shall provide regional services to families and individuals experiencing homelessness in Orange County.
2. Contractor shall provide rental assistance and housing relocation and stabilization services to individuals and families experiencing homelessness to secure permanent housing, increased stability, and connections to community resources and mainstream benefits. These include:
a. Rental application fees
b. Double security deposits
c. Utility deposits and payments
d. Holding fees
e. Move-in costs including truck rental, furnishings, moving of client’s belongings, payments to access storage, and other costs to move clients into housing.
f. Housing stability case management services through County-approved subcontracted service providers
3. Contractor shall serve a minimum of 17 households experiencing homelessness for the term of this Contract. Performance goals and thresholds are itemized in Section 2.B., with reporting and data requirements listed in Section 3 of this Scope of Services.
4. Contractor shall utilize and participate in the County Coordinated Entry System (CES) for families and individuals; meet CES requirements of 24 CFR part 576.400(d) or 24 CFR part 578.7(a)(8) and related HUD requirements.

1. Contractor shall provide regional services to families and individuals experiencing homelessness in Orange County.
2. Contractor shall provide rental assistance and housing relocation and stabilization services to individuals and families experiencing homelessness to secure permanent housing, increased income, and connections to public benefits and health services. These include:
a. Rental arrears for homelessness prevention b. Security deposits as needed

iii. Total number of persons experiencing homelessness to be served: The total number of persons experiencing homelessness to be served is 200 for the term of this Contract.
iv. Contractor shall provide temporary shelter, intake assessment, and case management services to individuals to secure permanent housing, increased income, and connections to public benefits and health services as set forth in 24 CFR 576.102. Such connections, as a result of the intake assessment and dependent upon the needs of the individual, may include, but are not limited to:

1. Contractor shall provide regional services described in 24 CFR 576.102 to individuals in families experiencing homelessness in Orange County.
2. Contractor shall provide temporary shelter, intake assessment, and case management services to individuals in families to secure permanent housing, increased income, and connections to public benefits and health services as set forth in 24 CFR 576.102. Contractor shall provide the foregoing services and connections in a commercially reasonable manner consistent with the requirements and standards set forth in CESH and this Contract, and as a result of the intake assessment and dependent upon the needs of the family household, including, but not limited to:
Program Essential Requirements. The Program shall meet the County’s need to provide emergency housing intervention services identified in this Contract to those experiencing homelessness through connection of services and an emergency shelter program.
1. Contractor shall provide regional services to families and individuals experiencing homelessness in Orange County.
2. Contractor shall provide temporary shelter, case management and supportive services to domestic violence victims experiencing homelessness to secure permanent housing, increased income, and connections to public benefits and health services.
3. Contractor shall have a target of serving 75 persons experiencing homelessness for the term of this Contract. Performance goals and thresholds are itemized in Section 2.B., with reporting and data requirements listed in Section 3 of this Scope of Services.
4. Utilize and participate in the County Coordinated Entry System (CES) for families and individuals; meet CES requirements of 24 CFR part 576.400(d) or 24 CFR part 578.7(a)(8) and related HUD requirements.
