Programmatic Reporting Requirements. Recipient will provide student and college data necessary to determine the impact of Xxxxxx. Data collected include, but are not limited to: student information forms, student activities surveys, official grades for each term, student update forms, and statistics regarding the college’s ethnic breakdown, retention/graduation rates, and transfer rates. Students will also participate in interviews, complete questionnaires, and/or complete other assessment instruments necessary to determine the outcome of Xxxxxx. In no case will data be collected which identifies individual students without a release form signed by the student. Data Collection Schedule: Because Xxxxxx staff coordinates data collection and reporting for both college and high school Xxxxxx programs, it is critical that the due dates be observed. Data is collected each year from the Xxxxxx Counselor or Xxxxxx English Instructor, and submitted to the Xxxxxx Statewide Office as designated below: Item Due Date Student Online Registration Oct 2020, 2021, 2022 Official Grade Sheet End of term
Programmatic Reporting Requirements. (a) ARPA Progress Reporting (due July 31, 2022; October 31, 2022; January 31, 2023): Using a Department-provided template, Subrecipient shall submit a progress report documenting performance of approved ARPA activities. (b) Final Report (due July 31, 2023): Subrecipient shall submit a final progress report using a Department provided template, which will be distributed by June 30, 2023.

Umoja and the College will establish a data collection methodology and schedule. The data to be collected shall include, but is not limited to: student information forms, student activity surveys, official grades for each term, student update forms, and statistics regarding the College’s ethnic breakdown, course completion rates, retention/persistence rates, graduation rates, and transfer rates. Students will also participate in interviews, complete questionnaires, and/or complete other assessments related to participation in Umoja programming with College. In addition to the foregoing, College must submit data as required by the California Community College Chancellor’s Office (CCCCO), to the MIS Special Populations database (SG08) within 30 days of the end of each academic term. The College will ensure that each Umoja student as described in Exhibit E is entered in the MIS Special Populations Database and confirm that the data is accurate via Datamart.

Programmatic Reporting Requirements. The subrecipient must submit programmatic reports using either the DPS Subgrant Progress Report Form or another format that includes all information required on the DPS form. The reporting periods are October 1 – December 31 (due January 30), January 1 – March 31 (due April 30), April 1 – June 30 (due July 30), and July 1 – September 30 (due October 30).

DODD will communicate final program report questions no later than June 1, 2024.
