PROJECT DESCRIPTION / PHYSICAL SCOPE. The Project, for which the provision of financial assistance is the subject of this Agreement, is hereby defined and described as follows: a) PROJECT NAME: Xxxxxxx Xxxxx Xxxxxxxx Park Natural Area Expansion b) SPECIFIC DESCRIPTION: The project consists of four parcels, 000-000-000000000000, 016-512- 100226013000, D16-512-100226017000 and 000-000-000000000000, all located within the Village of Pemberville at 000 Xxxxxxx Xxxxxx. Parcel D16-512-100226013000 will be a partial as the current property owner will retain ownership of the small approximately .17 acre parcel located adjacent to his residence. The appraisal was performed noting this was not included. c) PROJECT TYPE; MAJOR COMPONENTS: This project involves the fee simple acquisition of 32.81 acres of land consisting of approximately 7.2 acres of streamside forest, 23.1 acres of crop land and a 2.5 acre home site. Immediate plans are for the crop land to be planted to a native prairie to assist in filtering any adjacent runoff to the Portage River. The streamside forest will be managed to control invasive species. The 2.5 acres that presently surrounds the home site will be reduced with that area being included in the native prairie. The structures included in the home site will be utilized as staff offices and equipment storage. d) PROJECT EMPHASIS: Water quality; Open space planning and design. e) TERMS OF EASEMENTS N/A f) PUBLIC ACCESS INFORMATION The property is located adjacent to the Xxxxxxx Xxxxx Xxxxxxxx Park which is under the control of the Wood County Park District. The newly acquired acreage will be open to the public daily, 365 days of the year. g) OWNERSHIP MANAGEMENT OPERATION The property will be owned by the Wood County Park District, and will be incorporated into the management plan for park district-owned facilities maintained to established park district standards.

There are 2 deeds to the property. Parcel numbers: 171- 000027, 171-000565. 41.5 acres. c) PROJECT TYPE; MAJOR COMPONENTS: The project will acquire a significant tract of headwater corridor and forest along Xxxxxx Run, a primary tributary of Blacklick Creek, on the city's far east side. The 41.5 acre wooded parcel separates dense urban subdivision from the riparian corridor, and will form a critical headwaters preserve for the watershed. Site improvements include demolition and site clean up of the old home site. d) PROJECT EMPHASIS: Preserves headwater streams e) TERMS OF EASEMENTS N/A f) PUBLIC ACCESS INFORMATION CRPD initially plans to provide a nature trail loop around the site and the adjacent CRPD natural area, connecting over 3,500 residents within a 10-minute walk to the site. By a separate acquisition, the department will buy 4.5 acres of the former home site along Xxxxxxxx Road to be reserved as a space for a future small neighborhood park. g) OWNERSHIP MANAGEMENT OPERATION The site will be owned by the City of Columbus Recreation and Parks Department. CRPD Property management and Parks Maintenance Divisions will be responsible for ongoing management of the site. Debris from a former homestead on a small segment of the site will be demolished and removed. The Department will prepare a Land Management Plan for the site soon after acquisition.

Issues to be corrected are deficient deck, approach slabs, approach railing, bridge railing, parapets, painting, and drainage repair. See engineer's estimate in project application for approved bid items and quantities. d) PHYSICAL DIMENSIONAL CHARACTERISTICS: Four lane overpass to include two bridges approximately 456 ft in length for both brdiges. Maximum span for both bridges is 149 feet.

Project includes Xxxxx Road from Main Street to Horn Road, Maple Grove Road from SR 303 to Elementary School, Wilvern Drive from SR 303 to Xxxxxxx Xxxxx, Xxxxxx Xxxxxx from SR 303 to Xxxxxx Drive, and School Street from Xxxx Avenue to SR 303. c) PROJECT TYPE; MAJOR COMPONENTS: Basic repaving of asphalt roadway with "milling and filling" 2 inches of asphalt pavement, as well as full depth and partial depth repairs, and underdrain installed on certain sections as needed. New pavement markings will be applied to the new pavement and manholes adjusted to grade. See engineer's estimate in project application for approved bid items and quantities. d) PHYSICAL DIMENSIONAL CHARACTERISTICS: Roadway is variable width roadway between 18 to 24 feet wide with some section of rolled curb. This road is two lanes of residential streets. The total project length is 10,290 feet or 1.95 miles long.
