Special Inspection Agencies shall be chosen from the list of approved firms. BEFORE A PERMIT CAN BE ISSUED: The Owner of his/her representative, on the advice of the responsible Project Engineer or Architect, shall fill out completely, sign and submit two (2) copies of the attached “Structural Tests and Inspection Schedule” to this Department for review and approval. The Owner and General Contractor and Inspection/Testing Agency, where applicable, shall also acknowledge the following conditions applicable to Special Inspection and/or Testing.
SPECIAL INSPECTION AND TESTING AGREEMENT. SPECIAL INSPECTION AND TESTING AGREEMENT. In accordance with the Oregon Specialty Structural Code (OSSC) 107.1, the Registered Design Professional (RDP) shall prepare and submit a special inspection and structural observation program. The RDP shall confirm that the special inspection and structural observations noted below are indicated on the approved plans. This agreement is meant to denote general areas of construction requiring special inspection. Reference the approved plans for more specific requirements related to each respective area. Per OSSC 1704.2, the Building Official may accept all special inspectors registered with the Oregon Building Officials Association (OBOA) Special Inspection Program for the type of work for which they are certified/qualified to inspect. The Special Inspector shall observe and document the work for conformance with the building department approved plans and specifications and applicable workmanship provisions of the OSSC. Additionally, the special inspector(s), general contractor, owner and RDP shall comply with the duties and responsibilities outlined in Appendix D of the OBOA Special Inspection Program. Appendix D is available upon request or can be viewed at xxx.xxxxxxxxxxxxxxxxxxxxxxx.xxx. The following special inspections, material testing and structural observations shall be performed according to OSSC Chapter 17 unless a separate program of inspection is submitted by the RDP and approved by the Building Official. BEFORE A PERMIT CAN BE ISSUED: The RDP shall complete Part 2 of this agreement and verify all non- jurisdictional acknowledgements in Part 3 are completed. PRIOR TO RECEIVING FINAL INSPECTION: The Special Inspector or Inspection Agency and the RDP shall submit a Final Summary Report to the Building Official. The summary shall include a statement that all items requiring special inspection, testing or structural observations were fulfilled, reported and, to the best of their knowledge, in conformance with the building department approved plans, specifications, approved change orders and applicable workmanship provisions of the OSSC.

BEFORE A PERMIT CAN BE ISSUED: The owner or owner’s agent, the engineer or architect of record, and the Special Inspector shall complete two (2) copies of the attached Special Inspection and Testing Schedule including the requirement acknowledgments to the Building Division for review and approval. It is recommended that the contractor also sign the form if one has been selected for the project. A pre-construction conference with the parties involved may be required to review the special inspection requirements and procedures.

The Structural Tests and Inspection Schedule page is typically completed by the SEOR, however, Geocon understands that the purpose of submitting a completed copy of the City of Brentwood Special Inspection and Testing Agreement with this proposal is to indicate the services Geocon can perform. Geocon can retain a subcontractor to perform tasks marked with an (*), although these tasks are not typical for public-works construction projects. Items requiring continuous inspection and periodic inspection shall be so marked and clearly identified by the Project Engineer. When special inspection is required by CBC Section 1704 & 1705, the architect or engineer of record shall prepare an inspection program which shall be submitted to the Building Official per provisions of Section 1705. Structural Observations, if required, shall be provided per provisions of Section 1704. Special Inspection and Testing shall meet the minimum requirements of Section 1704 & 1705. SPECIAL INSPECTION AND TESTING AGREEMENT. Purpose: This Special Inspection and Testing Agreement (SITA) is a tool to define aspects of the project that require special inspection and testing as required in California Building Code 2019 Edition (2019 CBC) Sections 1704 through 1709, and to define duties and responsibilities for individuals and other parties connected with project.
SPECIAL INSPECTION AND TESTING AGREEMENT. When special inspections are required by Chapter 17 of the California Building Code (CBC), the RDP in responsible charge and RDP responsible for structural design shall prepare an inspection program (Part III), which shall be submitted to the Building Official for approval prior to issuance of the building permit. The special inspector shall be employed by the owner or an authorized agent of the owner, BUT NOT the contractor, or any other person responsible for the work. The special inspection firm(s) named above has(have) been authorized to perform the special inspection and testing services designated in this agreement, and in accordance with the CBC requirements, shall report all activities and results to the Building Official, and other parties as listed. It is understood that special inspections are required in addition to, and are not a substitute for, the normal inspections performed by the Building Inspector. The undersigned hereby affirm, under penalty of law that the special inspection program is in accordance with the requirements of the CBC and the City of Xxxxx. The undersigned has used all reasonable diligence in completing this form and to the best of his/her knowledge, the information contained herein is true and complete.
