Statement of Work/Program and Project Description. West Lakes must use these CDBG-CV funds towards its COVID-19 Food Relief Distribution program, provide PPE, and to support the salaries of two (2) COVID response individuals to enable them to provide the following services attached on Exhibit “D” to prevent, prepare for, and respond to coronavirus for approximately 500 low- and moderate-income persons, according to the CDBG income guidelines attached hereto (the “Project”). West Lakes must only spend these CDBG-CV funds in accordance with the Budget for this Project, which is attached as Exhibit “E” and made a part hereof by this reference. Additional requirements for the Project are attached hereto as Exhibit “F” and made a part hereof by this reference.

Statement of Work/Program and Project Description. JFS will use these CDBG funds to support the salaries, benefits and taxes of key personnel and staff who will provide case management services for approximately sixteen (16) low- and moderate-income persons according to the CDBG income guidelines attached hereto as Exhibit “A” (the “Project”). These services will focus on assisting the individual to overcome immediate crisis and achieve long-term financial stability and self-sufficiency. This goal is accomplished by providing FSP participants with case management, financial education, individual financial counseling, financial assistance for basic needs, employability skills training, mental health counseling, and food assistance, as needed. Additional requirements for the Project are attached hereto as Exhibit “C” and made a part hereof by this reference. The Budget for this Project is attached as Exhibit “D” and made a part hereof by this reference.
Statement of Work/Program and Project Description. CENTER administers and operates a community services program known as the Teen Parent Program, which is a program that that tackles the obstacles faced by the teen parent in achieving self-sufficiency by increasing life and parenting skills and ability to plan for and achieve future goals through the provision of case management, counseling, childcare, parenting education, post-school planning and other supportive services to parenting female youth (11-20) and their children (0-4) who reside in their 24-bed transitional shelter. Under this Program, CENTER shall serve approximately 25 pregnant or parenting young women and their children, who are City of Orlando residents and who are low- and moderate - income according to HUD income guidelines attached hereto as Exhibit “A” (hereinafter referred to as the “Project”). Additional CDBG program requirements for the Project are attached as Exhibit “B” and made a part hereof by this reference. The Budget for this Project is attached as Exhibit “C” and made a part hereof by this reference. During the term of this Agreement, CENTER agrees to work diligently towards the completion of the Project and complete the Project by September 30, 2015. If it is determined that CENTER will be unable to provide these services for the term of this Agreement, CENTER must submit a request for an extension to the City for consideration. This request must identify the reasons for the extension and must be accompanied by a proposed project timeline that can be reasonably accomplished. CENTER’s failure to work diligently toward completing the Project and incidents of non–performance may result in conditions being placed on the grant funds, suspension of grant funds, or the City may cease disbursing any other funds pursuant to this Agreement so that the City can reallocate the funds for other uses or projects.

CSCCF must only spend these CDBG-CV funds in accordance with the Budget for this Project, which is attached as Exhibit “E” and made a part hereof by this reference. Statement of Work/Program and Project Description. Coalition administers and operates the “First Steps Substance Abuse Recovery Program,” a four- to eight-month residential substance abuse recovery program for Central Florida homeless men who are low- to moderate-income. This FSSARP is for homeless men and is the largest recovery program of its kind in Central Florida and the only one to provide a full continuum of care specifically for homeless men and provides: meals, shelter, counseling, recovery meetings, life skills training, education, job skills training, employment placement and retention services, and financial literacy training. After completing the FSSARP, clients can be moved into the FSTHP to continue working on savings, education, job training or other challenges, if needed (hereinafter the FSSARP and the FSTHP programs are collectively referred to as the “Program”). Under this Program, Coalition will use these funds to serve twenty-five (25) qualified individuals who are low- to moderate-income according to HUD income guidelines attached hereto as Exhibit “A” (hereinafter referred to as the “Project”). Additional requirements for the Project are attached hereto as Exhibit “C” and made a part hereof by this reference. The Budget for this Project is attached as Exhibit “D” and made a part hereof by this reference.
