Accordingly, Customer acknowledges that Section 326 of the USA PATRIOT Act and Bank’s identity verification procedures require Bank to obtain information which may be used to confirm Customer's identity including without limitation Customer's name, address and organizational documents (“identifying information”). Customer may also be asked to provide information about its financial status such as its current audited and unaudited financial statements. Customer agrees to provide Bank with and consents to Bank obtaining from third parties any such identifying and financial information required as a condition of opening an account with or using any service provided by Bank.

BNPP NY, like all financial institutions, is required by Federal law to obtain, verify and record information that identifies each customer who opens an account with BNPP NY. When Customer opens an account with BNPP NY, BNPP NY will ask for Customer’s name, address, date of birth, government-issued identification number and/or other information that will allow BNPP NY to form a reasonable belief as to Customer’s identity, such as documents that establish legal status.

When Customer opens an account with BNPP PB, Inc., BNPP PB, Inc. will ask for Customer’s name, address, date of birth, government-issued identification number and/or other information reasonably required by BNPP PB, Inc. to form a reasonable belief as to Customer’s identity, such as documents that establish legal status.

To help the government fight the funding of terrorism and money laundering activities, federal law (including but not limited to the PATRIOT Act) requires all financial institutions (including Member Bank) to obtain, verify and record information that identifies each individual or business that opens an account, including any and all information provided by Merchant upon submission of the Merchant Application.

When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you. In accordance with Section 326 of the USA PATRIOT Act, signed October 26, 2001, Lafayette Federal Credit Union is required to obtain a copy of the documents used in identifying our new account members and in identifying signatory individuals added to an existing or new account. In some cases, identification will berequested for those individuals banking with the Credit Union prior to the effective date of the member identification requirements because original documentation was not obtained with the opening of the account or the Credit Union is unable to form a reasonable belief that it knows the true identity of the existing member. This Agreement covers your and our rights and responsibilities concerning Account(s) that Lafayette Federal Credit Union offers. In this Agreement, the words “you,” “your” and all variants mean anyone who signs a Membership Enrollment Form or otherwise opens an account at the Credit Union. The words “we”, “us,” “our” and “the Credit Union” mean Lafayette Federal Credit Union. The word “account” means any one or more savings or other accounts you have with the Credit Union. Your account type(s) and ownership features are designated on your Membership Enrollment Form. By signing a Membership Enrollment Form or completing or transmitting an on-line Membership Enrollment Form, each of you, jointly and severally, agree to the terms and conditions in this Membership and Account Agreement, Funds Availability Disclosure, Electronic Funds Transfer Disclosure, the Truth in Savings Terms and Conditions Disclosure, any account receipt accompanying this Agreement, the Credit Union’s Bylaws and policies, and any amendments of these documents from time to time that collectively govern your Membership and Accounts.

Xxxxxxx may also require a copy of the client’s driver’s license or other identifying documents. The Client represents that the monies comprising the Managed Assets are derived from legitimate sources and are not the result of or connected with any illegal activity.

Client consents to the Bank obtaining any such identifying information from third parties which may be required as a condition of opening Custody and Cash Accounts at Bank.
