Written Reprimand I. When a supervisor determines that a reprimand is warranted for unsatisfactory behavior, the supervisor will so notify the ESP and schedule a meeting for issuance of the Written Reprimand I. The reprimand will describe the specific inappropriate behavior involved, and the requirements for improvement. A copy will be placed in the ESP’s official personnel file, one copy will be given to the ESP, and one copy will be retained by the supervisor.

Written Reprimand I. If an employee's job performance is unsatisfactory, specific infractions(s) will be brought to their attention by their immediate supervisor, and an opportunity will be given them to address these issues. This reprimand will be followed by a written summary outlining the points covered and requirements necessary for the employee to improve. A copy will be placed in the employee's personnel file, and one copy retained by the supervisor.

  • Written Reprimand A written reprimand may be issued to an employee when appropriate in keeping with provisions of this Article. Such written reprimand shall be dated and signed by the supervisor/designee before it is provided to the employee. If the employee chooses not to sign to acknowledge receipt of the reprimand, it shall be witnessed to document that the employee received the same. Such reprimands shall be filed in the affected employee’s personnel file.

  • Written Request When Distributed - A Participant entitled to distribution who wishes to receive a distribution must submit a written request to the Plan Administrator. Such request shall be made upon a form provided by the Plan Administrator.
