Comprehensive care plan means an individualized person-centered plan described in He-E 805.05(c) that is:
Comprehensive care plan. (CCP) means a comprehensive plan of services that meets the Member’s Physical, Behavioral, and Long-Term Care needs. Comprehensive Needs Assessment (CNA) is an assessment of the Member’s Physical Health, Behavioral Health and Long-Term Care (LTC) needs; it will identify potential risks and provide social and cultural information. The results of the CNA will be used to create the care plan which is based on the Member’s assessed needs. The CNA may also include a functional assessment, if applicable.
