Comprehensive Needs Assessment. 4.4.4.5.1 The CONTRACTOR shall perform a CNA for each Member who has been determined as needing a CNA. As part of the CNA for Members who need Community Benefits, the care coordinator must conduct a NF LOC determination, administer the Community Benefits Services Questionnaire (CBSQ) and complete the Community Benefits Member Agreement (CBMA), and inform the Member of available Community Benefits. If the Member is enrolled in a Health Home, the CONTRACTOR shall follow the requirements in Section 4.13.2 of this Agreement. The CONTRACTOR may perform the HRA and the CNA concurrently.

The enrollment for Chefornak Schools hoovered around 140 students in SY 2021-2022.

The school, excluding Head Start, has 34 students enrolled and an additional. Attendance varies greatly family by family, but the majority of the students attend regularly. As a community, McGrath has a diverse population. There are families who are native to the area as well as many first generation Alaskan families. The school is very community oriented and offers opportunities throughout the year for community members to participate in different school events.

We have 18 general education classroom teachers, 6.6 specialist teachers, three special education resource teachers, two instructional coaches, one AP intern, one full time counselor and one intern, three extended resource classroom teachers, one bilingual teacher, eleven special education teaching assistants, three bilingual tutors, four kindergarten teacher assistants and one Indian Education tutor. We have a gifted program. 88% of our population is eligible for free and reduced lunch. The demographic break-down is as follows: 3% African American, 13% Alaska Native/American Indian, 36% Asian, 10% Caucasian, 9% Hispanic, and 16% are identified as belonging to two or more ethnic groups. The high rates of poverty and the growing population acquiring English language (51%) put our students at risk for achieving learning targets. Struggling learners in reading, writing and math are identified at the start of the year based on standards based assessments. Fast Bridge is used to progress monitor students. Teachers/special education/bilingual staff implement small group interventions based on these data. These services are provided in small group settings during literacy and math blocks. We use additional grant money to provide before and after school tutoring for identified students who are below proficient in reading or math.
