042 Reports. A. The Contractor must provide a field service report for each site visit, whether preventative maintenance or a repair visit. If parts are replaced, the Contractor must attach documentation to the field service report demonstrating that all parts replaced are original manufacturer parts. A copy of each field service report will be provided to the CCI identified in Section 2.022. B. Upon request, the Contractor must provide summary reports of preventative maintenance and repairs, including but not limited to: usage of spare parts by equipment serial number, type of repair by equipment serial number, and preventative and repair visits by equipment serial number. C. All reports shall be provided at no cost to the State.

042 Reports. The Contractor must submit written monthly summaries of progress which outline the work accomplished during the reporting period; work to be accomplished during the subsequent reporting period; problems, real or anticipated, which should be brought to the attention of the CCI; and notification of any significant deviation from previously agreed-upon work plans.

The Contractor must be able to generate claims status reports within 48 hours of request. c. The Contractor must provide detailed reports regarding number of claims paid and number of claims with an outstanding balance-separated by reason for rejection by insurance plan on a weekly basis. d. The Contractor must be able to provide reports indicating what adjustment information has been received and used on a weekly basis. e. The Contractor must save all insurance and Medicaid information for seven years and be able to provide it to MDCH, BOL electronically for archiving. f. The Contractor must provide any other reports requested by MDCH, BOL in a format and frequency agreed upon by the Contractor and MDCH, BOL.
042 Reports. The Contractor must provide written copy of the following reports as specified to the Contract Compliance Inspector for Xxxxxx Xxxxxxx Psychiatric Hospital. a. Written reports of specified routine x-rays are to be returned the following day by 8:00 AM. b. Significant abnormal reports should be telephoned and faxed to appropriate personnel as soon as tests are available. c. By the fifth (5th) of each month, an itemized statement of costs by procedure incurred in the proceeding month. d. The Contractor must furnish performance improvement monitoring reports to the Contract Administrator on a monthly basis. Reporting must be on mutually agreed content and format, to be agreed upon by the Contractor and the Project Manager when the Contract begins.
042 Reports. A. Reports are required for the one (1) 100% systems annual test and the yearly inspections. B. Reports for the sensitivity testing and cleaning of all system smoke detectors are required every two years. C. Reports for the two (2) annual tests and yearly inspections on the Kitchen Suppression System are required. D. Reports are required for the annual testing/inspections of the fire extinguishers. E. The awarded Contractor will provide a complete checklist with a detailed description clearly stating which devices will be tested and how they will be tested. F. The certificate of verification must be prepared and signed by a qualified representative of the equipment manufacturer, who witnessed the entire test and verification procedure. G. After all tests, the Contractor’s representative will certify that all systems are in working order and on standby mode. H. Upon completion of testing for each system, the Contractor must arm the system and leave it in 100% working order. The State’s representative will sign the Contractor’s test report to that effect.

042 Reports. The successful Contractor shall provide reports to the State for each service call, including the preventive maintenance and repair service calls. Upon request, the vendor must also provide usage reports of the number and type of service calls, and repair or maintenance spare parts usage.
042 Reports. Reporting formats must be submitted to the State’s Project Manager for approval within 15 business days after the execution of the contract resulting from this RFP. Once both parties have agreed to the format of the report, it shall become the standard to follow for the duration of the contract. Changes to reporting formats may be requested by the State or OPHP and will follow the standard guidelines for changes to the project/s.
