6.1.2 Employee’s requesting all types of leave including banked RDO’s must have an “Application for Leave” form completed by the Employee and submitted to the Employer for approval. With the exception of Personal Leave, Carers Leave, and scheduled RDO’s, leave is not to be considered approved unless the Employee has received an approved notification from the Employer.
