Sign-Up The Board shall provide each teacher with an application or enrollment form for participation in the medical, dental, extended health and group life insurance benefit plans. In the event a teacher does not wish to participate in any particular benefit plan where opting out is an option, the application or enrollment form must be so noted by the teacher and kept on file by the Board.

In the event that the carrier makes a unilateral change to the benefits over which District has no control, the District shall inform the Union. In these circumstances, the District shall not be held financially or otherwise responsible for the change. Changes in administration of the plans will be discussed with the Union prior to implementation. Any such changes will not affect the level of benefits provided herein.
