Client Eligibility. Client eligibility and service referral are the responsibility of DDA under chapter 388- 823 WAC (Eligibility) and chapter 388-825 WAC (Service Rules). Only persons referred by DDA shall be eligible for direct Client services under this Program Agreement. It is DDA’s responsibility to determine and authorize the appropriate direct service(s) type. Direct Client services provided without authorization are not reimbursable under this Program Agreement.

Client Eligibility a. OAA Title III
Client Eligibility. The Subrecipient shall establish, implement, and monitor policies and procedures to determine client eligibility based on each of the following three factors: (1) a documented diagnosis of HIV; (2) low-income; and (3) residency in the service area. Client Eligibility. Eligibility extends to any individual that has been the victim of sexual assault (primary and/or secondary victims). The primary presenting reason for an individual to receive services supported under this subcontract must be related to sexual assault. Domestic violence or other trauma-related services may not be supported with subcontract funds. However, if an individual presents as a victim of domestic violence or other trauma, but is also a victim of sexual assault, related sexual assault-specific victim services may be supported under this subcontract.
Client Eligibility. 3.4.1 The Network Service Provider shall provide services to individuals aged 0 to 25 who are:
3.4.1.1 In a crisis
a. The clinical threshold for crisis may include aggressive behaviors; suicide attempts/ideation; drug and alcohol overdose or abuse; or disruptive symptoms related to thought, mood and anxiety disorders (e.g., panic, hopelessness, anger, depression), escalating behavior(s) and, without immediate intervention, the individual is likely to require a higher intensity of services. It may also present as an overt change in functioning, or be prompted by traumatic life events. Mobile Response Teams must coordinate in- person services with law enforcement to provide additional safety, when appropriate and necessary.
3.4.2 The individual’s inability to pay for service must not be a criteria for eligibility.
Client Eligibility. All clients served under this agreement shall be qualified via either the HUD section 8 income verification or the HUD “presumed benefit” verification.
Client Eligibility. Individuals seeking services for this category shall have an eligibility established in Provide Enterprise. Eligibility shall demonstrate client eligibility as:
1. Low-income (below 80% of area median income)
2. Documented HIV/AIDS status (confidentiality must be maintained)
Client Eligibility. The target population is those who have complex placement needs that cannot be met in Alaska at all, or that must be resolved in a short timeframe while an appropriate placement in Alaska can be established. This population may include individuals who have been ordered to receive hospitalization for evaluation for civil commitment purposes but who have specialized needs for care that the State of Alaska cannot provide. This population could encompass all age groups, clinical presentations, and funding sources. This provider agreement does not apply to any services outlined under 7 AAC 53.300 – 7 AAC 53.370 in connection with children in state custody or under supervision.
Client Eligibility. Provider and Purchaser understand and agree that the eligibility of individuals to receive the services to be purchased from Provider under this Agreement will be determined by Purchaser. An individual is entitled to the right of a fair hearing concerning eligibility. Purchaser shall inform individuals of this right. Purchaser shall inform the client or guardian and the Provider in writing of a denial of eligibility. If a client requests a hearing, benefits shall continue until a decision is rendered.
