Dental Hygienist Scope of Work. Subcontractor shall provide adequate licensed personnel to perform dental services for students, including but not limited to: - Provide nine (9) hours of on-site dental hygienist services per week. Actual hours/days of on-site service shall be jointly agreed to between the CENTER, CONTRACTOR and DENTIST(S), and shall become a part of this contract. Unexpected and infrequent changes to the on-Center schedule may be made without consequence or need for amendment to this contract, as long as total hours are met each week. Frequent or extensive changes to the agreed upon schedule will be unacceptable. On-site dental hygienist services shall not be required during weekends, during CENTER holidays, or during the break periods - Perform preliminary oral examination; take bitewing and periodical X- Rays; - Perform prophies, scale and root plane; - Oral hygiene instructions including chair-side counseling on brushing, flossing and nutrition; - Education on hazards and risks of smokeless tobacco; - Process own instruments and tray set-ups; - Chart documentation (existing restorations, missing teeth, lesions), and; - Classifying occlusion - Work closely with the Wellness Director (WD) and Center Dentist(s) to ensure the previously listed services are provided to job corps trainees. - Collect data and prepare reports as required by the Center Director and/or the Department of Labor. - Cooperate with Corporate and Regional Office Center Assessments. - Maintain the accuracy and confidentiality of all required oral health and wellness records when they are in the subcontractor’s presence. - Provide support to the students in acquiring the oral health-related skills, knowledge, and attitudes that will make them employable. - Promote opportunities for the students to practice the skills that will help make them employable. - Comply with Job Corps infection control policies and procedures.

  • HOOSIER HEALTHWISE SCOPE OF WORK  Requirements for Reinsurance Companies  The Contractor shall submit documentation that the reinsurer follows the National Association of Insurance Commissioners' (NAIC) Reinsurance Accounting Standards.  The Contractor shall be required to obtain reinsurance from insurance organizations that have Standard and Poor's claims- paying ability ratings of "AA" or higher and a Xxxxx’x bond rating of “A1” or higher, unless otherwise approved by OMPP.  Subcontractors  Subcontractors’ reinsurance coverage requirements must be clearly defined in the reinsurance agreement.  Subcontractors should be encouraged to obtain their own stop-loss coverage with the above-mentioned terms.  If subcontractors do not obtain reinsurance on their own, the Contractor is required to forward appropriate recoveries from stop- loss coverage to applicable subcontractors.

  • General Scope of Work The Scope of Work and projected level of effort required for these SERVICES is described in Exhibit “A” attached hereto and by this reference made a part of this AGREEMENT. The General Scope of Work was developed utilizing performance based contracting methodologies.

  • School Improvement Plans The School shall develop and implement a School Improvement Plan as required by section 1002.33(9)(n), Florida Statutes and applicable State Board of Education Rules or applicable federal law.

  • Curriculum Work Service on a District curriculum committee shall be required for grade level or department chairpersons. All other teachers working on such committee(s) shall be designated (in writing) by the Curriculum Director and shall be paid, at the hourly rate contained in Schedule B, for all documented hours of committee service (up to such limits as may be imposed by the District) occurring when school is not in session (e.g. during the summer), during duty-free lunch periods or preparation periods, and before/after the teacher’s regularly scheduled work day. Curriculum committee work shall occur at such times as are determined or approved by the Curriculum Director.

  • School Improvement Plan As permitted under IC § 20-10.2-3-1.5, the Charter shall serve as the Charter School's strategic and continuous school improvement and achievement plan (hereafter, the "School Improvement Plan"). To the extent that IC § 20-10.2 applies to the Charter in its function as the School Improvement Plan, the Organizer shall comply with the requirements under IC § 20-10.2.
