DUTY TO REPORT CHILD ABUSE. The Parent is hereby advised that under the terms of the California Penal Code § 11166 the Center and its employees have a statutory duty to report any known or reasonably suspected instance of child abuse to a child protective agency. In addition, the Center and any employee who has knowledge of, or who reasonably suspects that mental suffering has been inflicted upon the Child, or that his or her emotional well-being is endangered in any other way, must report the known or reasonably suspected instance to a child protective agency.

DUTY TO REPORT CHILD ABUSE. Provider shall ensure that any employee who becomes aware of possible cases of abuse or neglect of any student, immediately reports such abuse or neglect to local law enforcement officials and to the Board. Under Ohio law, the duty to report applies when a person who is acting in an official or professional capacity knows or suspects that a child under eighteen (18) years of age or a mentally retarded, developmentally disabled, or physically impaired child under twenty-one (21) years of age as suffered or faces a threat of suffering any physical or mental wound, injury, disability, or condition of a nature that reasonably indicates abuse or neglect of the child.

Provider will not release the Child to anyone under the age of 18. Parent must sign the Child in/out upon arrival and departure each day. Parent must not remove the Child from day care home without notifying Provider. All persons other than parents picking up the Child will be required to provide their drivers license/ID and will be required to sign in with Time and full signature. ADDITIONAL CONSIDERATIONS Clothing: Parent should provide a change of clothing for the Child. All clothing must be labeled. Provider is not responsible for soiled or lost clothing.

Medications: All medication must be in the original container for the provider to dispense it. The following provisions also apply: Prescription Medication: Provider will administer prescription medication only so long as Parent has submitted to Provider (1) a signed permission form with instructions provided by parent, and (2) a prescription from the Child’s doctor with required dosage and times to be administered. Over the Counter Medication: Provider will not administer over the counter medication. Discipline: Provider will not use any corporal punishment. If discipline is required, Provider will use redirection and/or time-out. Medical Conditions/Allergies: Parent must fill out a form provided by Provider listing Childs allergies and all medical conditions.

DUTY TO REPORT CHILD ABUSE. The parent is hereby advised that staff members of the Child Care Center are mandated reporters and, as such, are required to report to the Department of Social Services any known or reasonably suspected instance of child abuse. The Department of Social Services shall have the authority to interview children or staff, and to inspect and audit child or faculty records without prior consent. The Center shall make provision for private interviews with any child or staff member, or for the examination of all records relating to the operation of the facility. The Department of Social Services shall have the authority to observe physical conditions of the child including conditions which could indicate abuse, neglect, or inappropriate placement, and to have a licensed medical professional physically examine the child.

Parents will be notified of any of these field trips and are always welcome to join us on our field trips. If you wish for your child/children not to participate, please inform the daycare (Parents will be responsible for care for that day). ▪ Please note that during the summer we will be often taking the children in our care to the Forest Grove Park, located just one block from our daycare home. If in these occasions we are NOT able to notify the parents in advance, we will leave a note at our door. • ARTICLES TO BE SUPPLIED BY PARENTS • At least one change of clothes, including socks, and underwear. • Both shorts and pants, except during the winter. • In the summer, swimsuit (or swimmers), sunscreen and bug spray. • Diapers / Training pants, creams, wipes and toothbrush. • Bottles, baby food, formula, etc. (if needed). • Weather appropriate clothing for outdoor play and walks. • Car Seat or Booster Seat (if needed). Note: In the spring, summer and fall parents should ensure their child/children has/have sunscreen on when they arrive, everyday. These pictures are also used for kid’s crafts sent to parents on special occasions such as birthdays and holidays. Parents who do NOT wish to have photos or videos of their child (children) taken, MUST write a note and sign it in the space provided below. PHOTOGRAPHS & VIDEOS ▪ Information Disclosure • All parents’ and children’s information given to Jiboia Daycare will be kept confidential and will NOT be released to anyone, unless in case of emergency when requested by the child’s emergency contact, police, fire department and/or medical personnel.
