Employee Monthly Premium Cost for Active Employees. Upon ratification and approval, all current (active) employees and employees hired on or after ratification will contribute twelve percent (12%) of the cost of the Health Insurance, which includes prescription drugs, for either single or family coverage in the HDHP, the POS 298 (205) or a replacement plan. The Company will establish a debit account (105H) of three thousand dollars ($3,000), for a family plan and refill that amount each year thereafter to a maximum of three thousand dollars ($3,000). Example: Employee utilizes two thousand dollars ($2,000) from 1-1-20 through 12-31-20, two thousand dollars ($2,000) will be placed in the 105H accounts on 1-1-15 totaling three thousand dollars ($3,000). For a single plan fifteen hundred dollars ($1,500) will be placed in the employee’s account and refilled each year thereafter, to a maximum of fifteen hundred dollars ($1,500). Example: Employee utilizes one thousand dollars ($1,000) from 1-1-20 through 12-31-20, one thousand dollars ($1,000) will be placed in the 105H account on 1-1-21 totaling fifteen hundred dollars ($1,500).
Employee Monthly Premium Cost for Active Employees. Upon ratification and approval the company will pay the full monthly premium cost for either single or family coverage in the Traditional Blue POS 298 (POS 203) plan until April 1, 2018, thereafter effective April 1, 2018, all active and new hires shall contribute ten per cent (10%) of the monthly cost for either single or family coverage for the Traditional Blue POS 298 (POS 205) plan or a replacement plan selected by the company under section 7.5 below. Effective April 1, 2018, and thereafter until a successor agreement is reached, any current or post ratification employee enrolled in the POS 298 (POS 205) plan shall not be required to pay greater than $240 per month for the monthly premium or equivalent cost established by Blue Cross Blue Shield for such coverage.

Effective April 1, 2015, or as soon as practicable thereafter, all currently (active) employees and employees hired or promoted into Teamsters Local 264 (Police Lieutenants and Captains) will contribute ten (10%) percent of the monthly premium cost for either single or family coverage on the HDHP or replacement plan. The Company will establish a debit account (105h) of three thousand dollars ($3,000) for a family plan and replenish that account each year thereafter on January 1st to a maximum of three thousand dollars ($3,000). For a single plan, fifteen hundred dollars ($1,500) will be placed in the employee account and refilled once each year thereafter on January 1st, to a maximum of fifteen hundred dollars ($1,500).

Example: Employee utilizes $2,000 from 1-1-16 through 12-31-16. $2,000 will be placed in the 105H account on 1-1-17 totaling $3,000. For a single plan fifteen hundred dollars ($1,500) will be placed in the employee account and refilled each year thereafter, to a maximum of fifteen hundred dollars ($1,500). Example: Employee utilizes $1,000 from 1-1-16 to 12-31-16, $1,000 will be placed in the 105H account on 1-1-17 totaling $1,500.
