Family Transportation Benefit. (i) When an Insured Person is confined as an in-patient in a hospital due to Accidental Bodily Injuries that result in a Loss, Allianz will pay for the expenses incurred to transport an Immediate Family Member of the Insured Person to the hospital. Such personal attendance must be recommended by an attending physician, and such transportation must be via Common Carrier on the most direct route available.
(ii) When an Insured Person’s loss of life results in a loss of life benefit amount being payable, Allianz will pay for the expenses incurred by an Immediate Family Member of the Insured Person for transportation to the place where the Insured Person’s body is located for the purpose of identifying the Insured Person’s body. Such transportation must be via Common Carrier on the most direct route available. The maximum Family Transportation Benefit payable is $5,000 per Insured Person who is hospitalized as described above.
Family Transportation Benefit. Business Travel Accident Benefit for an amount equal to two (2) times base salary, subject to a maximum coverage level of $500,000. All benefit provisions are subject to the terms and conditions set forth in the Plan Certificates.

Such personal attendance must be recommended by an attending physician, and such transportation must be via Common Carrier on the most direct route available.
Family Transportation Benefit. We will reimburse the actual expenses incurred in transporting one Immediate Family Member from the Insured Person’s residence to the Hospital where the Insured Person is admitted, provided that
(a) We have accepted a Claim for In-patient Treatment under the Policy in respect of the same Hospitalisation;
(b) Such Hospital is located at least 150kms away from the Insured Person’s residence. The payment under this benefit is over and above the Basic Sum Insured subject to limits specified, if any. Note: In this Benefit, Immediate Family Member means the Insured Persons including Self and Group members as defined in the Policy Schedule/ Certificate of Insurance
Family Transportation Benefit. Business Travel Accident Benefit for an amount equal to two (2) times base salary, subject to a maximum coverage level of $500,000. The University will provide the option for full-time and Group I part-time bargaining unit members to purchase, on an after-tax basis, voluntary group term life insurance for themselves, their spouse, and their eligible dependent children. All benefit provisions are subject to the terms and conditions set forth in the Plan Certificates.
Family Transportation Benefit. If an injury results in the insured person being confined as an in-patient in a hospital more than 150 kilometers from his or her city of permanent residence, and requires personal attendance of an immediate family member, as recommended by a doctor in writing to us, we will reimburse the expense incurred by the immediate family member, for the transportation by the most direct route by a licensed common carrier to where the insured person is confined. This benefit cannot exceed the maximum amount shown in the policy schedule.
Family Transportation Benefit. If you are hospitalized more than 150 kilometres from your home as a result of an injury for which benefits are payable under this benefit provision, Great-West Life will pay the actual expense incurred less any amount paid for the same expenses under this plan’s global medical assistance benefit, up to $2,000, for transportation and lodging expenses for one family member to join you. Benefits for lodging are limited to moderate quality accommodation for the area of hospitalization. Telephone expenses and taxicab and car rental charges are included. Meal expenses are not covered. Transportation expenses are limited to round trip economy class transportation. If a private vehicle is used, expenses are limited to $.44 per kilometre travelled. If benefits are payable under this benefit provision for your death, Great-West Life will pay for expenses associated with your spouse’s enrolment in an accredited occupational training program. The purpose of the training program must be to provide the spouse with at least the minimum qualifications required for employment in an occupation for which the spouse would not otherwise qualify. Great-West Life will pay up to 10% of the Principal Sum, or $10,000, whichever is less. No benefits will be paid for expenses incurred more than 3 years after the accident causing your death, or room or board or other ordinary living, travelling, or clothing expenses.
Family Transportation Benefit. Pays up to for hotel accommodations for a family member if you are hospitalized more than kilometres away from home.
Family Transportation Benefit. If, as a result of Injuries covered under the Group Policy: • an Insured Person is confined to a Hospital located more than 200 kilometers from his or her normal place of residence, within 365 days of the date of the accident; and • the attending Doctor recommends the personal attendance of a Member of the Immediate Family of the Insured Person, we will pay the actual expenses, to a maximum of $10,000, incurred by such Member of the Immediate Family for transportation, to the Insured Person, by a licensed common carrier.
